超声引导下穿刺联合促性腺激素释放激素类似物治疗复发性子宫内膜异位囊肿的效果及对妊娠的影响

摘　　要：目的探讨超声引导下穿刺联合促性腺激素释放激素类似物(Gn RHa)治疗复发性子宫内膜异位囊肿的临床效果及对妊娠的影响。方法回顾性分析2013年1月至2014年12月本院诊治的100例复发性子宫内膜异位囊肿患者的临床资料,按照治疗方案不同分为两组,对照组(44例)患者未作任何处理,研究组(56例)患者在超声引导下穿刺联合Gn RHa治疗,比较两组患者的疗效、激素水平变化、健康调查简表(SF-36量表)评分及妊娠率。结果研究组患者治疗总有效率明显高于对照组(P<0.01)。研究组患者雌二醇(E2)、黄体生成素(LH)水平高于对照组,促卵泡生成素(FSH)水平低于对照组(P<0.01)。两组患者治疗后SF-36量表评分均明显高于治疗前(P<0.01),且研究组患者治疗后评分明显高于对照组(P<0.01)。研究组患者妊娠率明显高于对照组(P<0.05)。结论超声引导下穿刺联合Gn RHa治疗复发性子宫内膜异位囊肿效果显著,有利于改善患者激素水平,提高其生活质量及妊娠率。Objective To explore the effect of puncture guided by ultrasound combined with gonadotropin releasing hormone analogue （GnRHa） in the treatment of recurrent endometrial implantation cyst and its effect on pregnancy. Method The clinical data of 100 patients with endometrial implantation cyst in our hospital from January 2013 to December 2014 were reviewed, and they were divided into two groups according to the different treatments. 44 cases of postoperative recurrence of endometrial implantation cyst without any treatment were included in control group, 56 cases treated with puncture guided by ultrasound combined with GnRHa were included in study group. Curative efficacy, hormone levels, SF-36 scale score and pregnancy rate between the two groups were compared. Result The total effective rate of study group was significantly higher than control group （P 〈 0.01）; the levels of E2 and LH in study group were higher than control group, and the level of FSH was lower than control group （P 〈 0.01）. The scores of SF-36 scale after treatment in the two groups were significantly higher than before treatment （P 〈 0.01）. The scores of SF-36 scale in study group were significantly higher than control group （P 〈 0.01）. The pregnancy rate in study group was significantly higher than control group （P 〈 0.05）. Conclusion The effect of puncture guided by ultrasound combined with GnRHa in the treatment of recurrent endometrial implantation cyst is significant, can improve the level of hormone and the quality of life and pregnancy rate.
