咽喉反流患者鼻腔分泌物中胃蛋白酶的检测及意义

摘　　要：目的探讨鼻腔分泌物中胃蛋白酶的检测对咽喉反流的诊断价值。方法入选87例慢性鼻-鼻窦炎患者和31例健康志愿者,分别设为病例组和对照组,采用症状指数量表(RSI)、反流检查计分表(RFI)联合诊断性治疗实验(三联诊断)对咽喉反流进行诊断,酶联免疫吸附法(ELISA)检测鼻腔分泌物中胃蛋白酶含量,对比观察胃蛋白酶和三联诊断对咽喉反流的诊断价值。结果 RSI、RFI、诊断性治疗实验单项及联合诊断病例组中咽喉反流阳性率分别为68.97%、71.26%、72.41%、75.86%,其中RSI、RFI单项及联合诊断诊断阳性率显著高于对照组中,差异有统计学意义(P<0.05);以胃蛋白酶诊断咽喉反流,病例组阳性率为80.46%,显著高于对照组的25.81%,差异有统计学意义(P<0.05);胃蛋白酶检测与三联诊断咽喉反流一致性良好(P<0.05),以三联诊断咽喉反流作为标准,胃蛋白酶诊断咽喉反流的敏感度为95.45%,特异度为66.67%,阳性预测值为90.00%,阴性预测值为82.35%。结论鼻腔分泌物中胃蛋白酶的检测对咽喉反流具有较高的诊断价值,值得临床积极推广。Objective To explore the diagnosis value of nasal secretions pepsin to throat reflux. Methods Chose 87 cases patients with chronic nasal sinusitis and 31 cases healthy volunteers, they were respectively set as case group and the control group,they were diagnosed by Symptom index （RSI）,reverse flow check the score table （RFI） combined with diagnostic experimental treatment （joint diagnosis）, detected the pepsin in nasal secretions by enzyme-linked im- munosorbent （ELISA） ,observed the diagnosis value between sanlian diagnosis and pepsin. Results The throat reflux positive rate in case group diagnosed by RSI,RFI,diagnostic treatment experiment and joint diagnosis were respectively 68.97 %, 71.26 %, 72.41 %,71.26 %, the throat reflux positive rate in case group diagnosed by RSI, RFI joint diagnosis were respectively higher than in control group,the difference was statistically significant （P〈0.05） ;With pepsin diag- nosis of laryngeal reflux,the throat reflux positive positive rate was 80.46 %,it＇s significantly higher than that of con- trol group ,the difference was statistically significant （P〈0.05） ;Pepsin detection and the joint diagnosis throat had a good reflux consistency （P〈0.05）, with the joint diagnosis throat reflux as standard, pepsin sensitivity in the diagnosis of pharyngeal reflux was 95.45%, 66.67%, positive predictive value was 90.00% and negative predictive value was 82.35 %. Conelusion The diagnosis value of nasal secretions pepsin to throat reflux is high,it worth clinical positive promotion.
