256层CT口服法小肠造影诊断小肠肿瘤的临床价值

摘　　要：目的评价256层螺旋CT口服法小肠造影在诊断小肠肿瘤的价值。方法选取216例小肠肿瘤疑患者,男96例,女120例,男女性别比约为4∶5;年龄38~75岁,平均（（57.89±11.13）岁,口服2.5%甘露醇约2 000ml,行256-MSCT平扫及增强检查,通过多平面重建观察病变部位、大小、形态、周围侵犯及转移情况,得出CT诊断结果,将结果与最终临床病理结果进行对照分析。结果本研究选取的216例小肠肿瘤疑患者中,60例MSCTE诊断为小肠肿瘤,并与最终病理结果相符。156例MSCT诊断为阴性,其中144例相符,12例漏诊。通过统计学分析计算得出256-MSCT口服法小肠造影诊断小肠肿瘤的敏感度为83.3%[60/（60＋12）],特异度为100%[144/（0＋144）],正确率为94.4%[（60＋144）/（60＋0＋12＋144）],阳性预测值为100%[60/（60＋0）],阴性预测值为92.3%[144/（12＋144）]。结论 256-MSCT口服法小肠造影诊断小肠肿瘤的敏感度、特异度、正确率、阳性预测值及阴性预测值均较高,具有重要的临床价值。Objective To evaluate the value of 256-slice CT angiography in the diagnosis of small bowel tumors by oral enteroclysis.Methods Totally 216 patients with suspected small bowel tumor disease were collected as research subjects,including 96 males and 120 females,male to female ratio was about 4∶5,aged 38 to 75 years,mean age 57.89±11.13 years old.The patients were given oral 2.5% Manitol about2000 ml,lined 256- MSCTE plain and enhanced inspection,and lesion location,size,shape,surrounding violations and metastasis were observed by MPR,CT diagnostic results were obtained and were compared with the final clinic pathological results.Results This study selected 216 cases of patients with suspected small bowel tumors,60 cases MSCTE were diagnosed as intestinal tumor,and in line with the final pathology results.156 cases were diagnosed as negative MSCT,which matches the 144 cases,12 cases of misdiagnosis.Calculated by statistical analysis angiography in the diagnosis of small bowel tumors by the 256-MSCT with oral enteroclysis,the sensitivity,specificity,correct rate,positive predictive value,and negative predictive value were 83.3% [60/（60＋12）],100% [144/（0＋144）],94.4% [（60＋144）/（60＋0＋12＋144）],100% [60/（60＋0）]and 92.3% [144/（12＋144）]respectively.Conclusions 256- MSCT angiography with oral enteroclysis in the diagnosis of intestinal tumor has important clinical value with a higher sensitivity,specificity,accuracy,positive predictive value and negative predictive value.
