激素经导管法咽鼓管吹张治疗分泌性中耳炎

摘　　要：目的观察导管法咽鼓管吹张经空气压缩泵雾化给药(糖皮质激素)治疗分泌性中耳炎的效果。方法收集2012年11月到2013年12月来我院门诊就诊且无鼻腔疾病的分泌性中耳炎患者86例,随机分成两组每组43例,实验组给予糖皮质激素(布地奈德混悬剂)通过导管法咽鼓管吹张经空气压缩泵雾化器吹入患者鼓室内,对照组仅给予传统导管法咽鼓管吹张,两组均口服桉柠蒎肠溶胶囊。结果激素吹张组(实验组)中痊愈36人占83.7%,好转5人占11.6%,无效2人占4.7%。传统导管法咽鼓管吹张组(对照组)中痊愈25人占58.1%,好转14人占32.6%,无效4人占9.3%。结论导管法咽鼓管吹张经空气压缩泵雾化器将药物(糖皮质激素)送入鼓室内可有效治疗分泌性中耳炎,具有治疗安全、有效、费用低、疗程短等优点。Objective To report therapeutic effects of Eustachian tube catheterization and insufflation with inhalation of atomized glucocorticoid via an air compressor pump for the treatment of secretory otitis media （SOM）. Methods Eighty six patients with secretory otitis media and no nasal cavity diseases seen from November 2012 to December 2013 were divided into two groups randomly （43 patients each）. Patients in the study group were treated with Eustachian tube insufflation with inhalation of atomized glucocorticoid （Budesonide suspension） via an air compressor pump, while those in the control group received conventional Eustachian tube catheterization and insufflation. Both groups were treated with additional oral Eucalyptus Lemon Pinene Enteric Capsules. Results In the study group, SOM resolved in 83.7% （36/43） of patients, improved in 11.6% （5/43） of patients and remained unchanged in 4.7% （2/43） of patients. In the control group, SOM resolved in 58.1% （25/43） of patients, improved in 32.6% （14/43） of patients and remained unchanged in 3% （4/43） of patients. The differences in overall effectiveness between the two groups were statistically significant （P 〈 0.05）. Conclusion Eustachian tube catheterization and insuffiation with inhalation of atomized glucocorticoid via an air compressor pump into the tympanic cavity is an effective treatment for SOM. It is safe and inexpensive, and requires a relatively short course of treatment.
