808例社区老年高血压病患者中医体质状况与相关因素分析

摘　　要：目的观察社区老年高血压病患者中医体质状况及相关因素。方法采用多阶段分层抽样的方法,在上海某辖区内选取3个社区,再应用随机抽样方法,在每个社区分别抽取2个居委,招募抽取居委中的老年高血压病患者实施调查。收集研究对象的一般人口学资料、中医体质特征和行为生活方式相关数据,分析中医体质特征及与相关因素的关系。结果被调查的808例社区老年高血压病患者的偏颇体质以气虚质(28.59%)、阳虚质(22.03%)、阴虚质(17.70%)和痰湿质(16.21%)为主。不同性别患者在平和质、气虚质、血瘀质、气郁质和特禀质上的差异有统计学意义(P<0.05),其中平和质在男性中所占的比例相对较高,气虚质、血瘀质、气郁质和特禀质在女性中所占的比例相对较高。平和质在不吸烟者中所占的比例较高,而气虚质、阴虚质和气郁质在吸烟者中所占的比例较高(P<0.05)。男性体重与痰湿质呈正相关,女性体重与阳虚质呈负相关(P<0.05)。男性腰围与气虚质、痰湿质、湿热质和气郁质呈正相关(P<0.05)。体质指数与阳虚质呈负相关,与痰湿质、湿热质呈正相关(P<0.05)。高血压病患者的服药依从性与各中医体质均有相关关系(P<0.001),服药依从性越好,平和质得分则越高,其他偏颇体质得分则越低。结论社区老年高血压病患者的偏颇体质以气虚质、阳虚质、阴虚质和痰湿质为主,性别、吸烟行为和服药依从性是影响体质状况的主要因素。Objective To explore Chinese medicine constitution status and related factors of elderly hypertension community-based patients.Methods Adopt multistage stratified sampling method.Select 3 communities in Shanghai.Then randomly extract 2 residents groups in each community respectively to collect general demographic data and related data of Chinese medicine constitution characteristics and behavior life style of research objects.Analyze the relationship between Chinese medicine constitution characteristics and related factors.Results Abnormal constitution of 808 investigated elderly hypertension community-based patients was mainly Qi deficiency constitution（28.59%）,Yang deficiency constitution（22.03%）,Yin deficiency constitution（17.70%）and phlegm-dampness constitution（16.21%）.The difference of different gender patients of normal constitution,Qi deficiency constitution,blood stasis constitution,Qi stagnation constitution and specific diathesis constitution was significant（P〈0.05）.The proportion of normal constitution was larger in male,while the proportion of Qi deficiency constitution,blood stasis constitution,Qi stagnation constitution and specific diathesis constitution was larger in female.The proportion of normal constitution was larger in non-smokers,while the proportion of Qi deficiency constitution,Yin deficiency constitution and Qi stagnation constitution was larger in smokers（P〈0.05）.There was a positive correlation between weight and phlegm-dampness constitution in male subjects.There was a negative correlation between female weight and Yang deficiency constitution in female subjects（P〈0.05）.There was a positive correlation between waist circumference and Qi deficiency constitution,phlegm-dampness constitution,dampness-heat constitution and Qi stagnation constitution in male（P〈0.05）.While there was a negative correlation between body mass index（BMI）and Yang deficiency constitution,a positive correlation between BMI and phlegm-dampness constitution and dampnesshea
