单剂量右美托咪定用于成人全身麻醉后苏醒期躁动35例疗效评价

摘　　要：目的比较单剂量右美托咪定和芬太尼用于成人全身麻醉后苏醒期躁动的疗效。方法选择70例气管插管全身麻醉下行下腹部手术治疗并发术后苏醒期躁动患者,随机均分为两组。对照组应用芬太尼,观察组应用单剂量右美托咪定。结果观察组和对照组治疗后Riker镇静躁动评分(SAS)分别为(3.62±0.79)分和(4.65±0.94)分,均较治疗前有显著减少,且组间差异有统计学意义(P<0.05);观察组平息躁动时间和丙泊酚用量分别为(13.94±1.44)min和(68.73±4.09)mg,与对照组的(25.34±1.78)min和(220.73±11.49)mg比较均明显减少(P<0.05);治疗后观察组T1,T2,T3时收缩压(SBP)和心率(HR)均较T0显著降低,且两组间同时间点比较,差异有统计学意义(P<0.05)。结论单剂量右美托咪定用于治疗成人全身麻醉后苏醒期躁动疗效确切,值得临床推广。Objective To compare the effect of single dose dexmedetomidine and fentanyl for the treatment of adult emergence agitation after general anesthesia. Methods 70 cases of patients with hypogastric operation underwent tracheal intubation general anesthesia combined with emergence agitation were selected and randomly divided into two groups. 35 cases of patients in the control group used fentanyl, while 35 cases of patients in the study group used single dose dexmedetomidine. Results After treatment, the RSS score in the observation group and the control group after treatment were（ 3. 62 ± 0. 79） and（ 4. 65 ± 0. 94）, which were significantly decreased than before treatment, and there was a statistically significant difference between the two groups（ P〈0. 05）; the agitation calming time and the dosage of propofol in the observation group were（ 13. 94 ± 1. 44） min and（ 68. 73 ± 4. 09） mg, which were significantly fewer than（ 25. 34 ± 1. 78） min and（ 220. 73 ± 11. 49） mg of the control group（ P〈0. 05）; SBP and HR in the observation group after treatment at T1, T2,T3 were significantly lower than T0, and the two groups had statistically significant difference（ P〈0. 05） at the same time point. Conclusion Single dose of dexmedetomidine has a definite efficacy in treating adult emergence agitation after general anesthesia, which is worthy of clinical reference promotion.
