血清免疫固定电泳和游离轻链检测在多发性骨髓瘤的诊断价值

摘　　要：目的探讨多发性骨髓瘤(MM)患者血清免疫固定电泳(IFE)和游离轻链(sFLC)检测的临床价值。方法对60例临床确诊MM患者进行IFE分析和sFLC水平检测,并对结果进行分析。结果血清IFE分析结果:IgG型占55.00%(33/60),IgA型占18.33%(11/60),IgM型占8.33%(5/60),单纯轻链型占18.33%(11/60)。sFLC检测结果:35例κ型MM患者的sFLC-κ水平和25例λ型MM患者的sFLC-λ水平均高于正常参考范围(P<0.01);其sFLC-κ/λ比值分别>1.65和<0.26。血清IFE分析与sFLC检测结果完全符合。结论血清IFE分析和sFLC检测在MM的诊断与分型、病情监测、疗效评估和指导治疗等方面具有重要的临床价值。Objective To explore the clinical significance of serum immunofixation electrophoresis（IFE） and free light chain（sFLC） in the patients with multiple myeloma（MM）. Methods The detection and analysis of IFE and sFLC were performed in 60 patients diagnosed as MM. Results IFE analysis showed that immunophenotype IgG accounted for 55.00% （33/60）, IgA for 18.33% （11/60）, IgM for 8.33% （5/60） and pure light chain type for 18.33% （11/60）, respectively. The results of sFLC analysis indicated that the sFLC-κ levels in κ-MM patients（35 cases） and the sFLC-X levels in λ-MM patients（25 cases） were significantly higher than those in normal controls（P〈0. 01）. The ratio of sFLC-κ/λ was 〉1.65 in κ-MM patients and 〈0. 26 in λ-MM patients. The results of IFE analysis were completely accordance with those of sFLC determination. Conclusion The IFE analysis and sFLC determination have important clinical values in the diagnosis and classification, illness monitoring, guidance of treatment and evaluation of efficacy in the patients with MM.
