InterTan系统与第三代Gamma钉治疗不稳定转子间骨折的对比研究

摘　　要：目的比较联合加压交锁髓内钉系统与第三代Gamma钉治疗不稳定型转子间骨折的疗效。方法选取2010年3月至2012年10月收治不稳定型转子间骨折133例,根据自愿原则分为联合加压交锁髓内钉组63例和第三代Gamma钉组70例,手术后随访一年观察疗效。结果第三代Gamma钉组手术时间短于联合加压交锁髓内钉组[(80.4±11.1)min vs.(89.3±17.2)min,P<0.05];2组患者术中失血量、术后疗效及股外侧疼痛发生例数差异均无统计学意义。结论第三代Gamma钉和联合加压交锁髓内钉均可安全、有效治疗不稳定型转子间骨折。第三代Gamma钉操作相对简单,联合加压交锁髓内钉理论上具有更高的稳定性。Objective To compare the clinical efficacy of Intertrochanteric Antegrade Nail and third generation of Gamma nail for the treatment of unstable intertrochanteric fractures. Methods From March 2010 to October 2012, total of 133 patients with unstable intertrochanteric femoral fracture were prospectively divided into the intertrochanteric antegrade nail group（n=63）and the third generation of Gamma nail group（n=70）. Patients were followed prospectively for one year. Results The mean operative time of the third generation of Gamma nail group was significantly shorter than that of the inter- trochanteric nail group（P〈0.05）. There were no statistically significant differences between the two groups regarding the volume of blood loss, mean postoperative Parker scores of hip and pain of lateral thigh. Conclusion Unstable intertrochanteric fracture can be treaded successfully with Inter Tan and the third generation of Gamma nail.
