摘 要:目的初步探讨klotho在慢性肾脏病(CKD)分期及其并发继发性甲状旁腺功能亢进症(SHPT)中的作用。方法选取2012—2014年在海南省人民医院肾病风湿科门诊或住院的符合纳入与排除标准的CKD患者85例为研究对象。根据CKD分期标准,将患者分为CKD1~2组(26例)、CKD3~4组(34例)和CKD5组(25例)。CKD3~5期的患者根据甲状旁腺素(PTH)水平分为非SHPT组(n-SHPT组,PTH≤120 ng/L,26例)和SHPT组(PTH〉120 ng/L,33例)。收集患者的基本资料(性别、年龄、体质量、血压),检测其常规血清学指标〔血红蛋白(Hb)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、清蛋白(ALB)、尿素氮(BUN)及血肌酐(Scr)、钙、磷、碱性磷酸酶(ALP)、肾小球滤过率(GFR)、PTH〕和可溶型klotho(s-kl)、25-羟基维生素D(25-OH-D)、成纤维细胞生长因子23(FGF-23)水平。结果 CKD3~4组、CKD5组平均动脉压(MAP)、Hb、BUN、Scr、钙、磷、ALP、PTH与CKD1~2组比较,差异有统计学意义(P〈0.05);CKD5组ALT与CKD1~2组比较,差异有统计学意义(P〈0.05);CKD5组Hb、ALT、AST、BUN、Scr、钙、磷、ALP、PTH与CKD3~4组比较,差异有统计学意义(P〈0.05)。CKD3~4组、CKD5组s-kl、25-OH-D、FGF-23水平与CKD1~2组比较,差异有统计学意义(P〈0.05);CKD5组s-kl、25-OH-D、FGF-23水平与CKD3~4组比较,差异有统计学意义(P〈0.05)。n-SHPT组、SHPT组ALB、BUN、Scr、钙、磷、ALP、GFR、s-kl、25-OH-D水平比较,差异有统计学意义(P〈0.05)。s-kl与钙、GFR、25-OH-D呈正相关,与磷、ALP、PTH呈负相关(P〈0.05);s-kl与FGF-23无直线相关关系(P〉0.05)。PTH水平影响因素的直线回归分析结果显示,钙进入回归方程,回归方程为y=334.20-92.02x(P〈0.05)。结论 klotho水平随着CKD进展逐渐下降,其在CKD及其合并SHPT患者中降低可能是促进CKD进展和钙代谢失调,进而导致SHPObjective To make a preliminary investigation on the effect of klotho on chronic kidney disease ( CKD ) staging and secondary hyperparathyroidism (SHPT) . Methods From 2012 to 2014, 85 CKD inpatients and outpatients who accorded with the inclusion and exclusion criteria from the Department of Nephrology & Rheumatology of Hainan General Hospital were enrolled in the study. According to CKD staging criteria, divided the patients into three groups: CKD1 -2 group (n = 26) , CKD3 -4 group (n = 34) and CKD5 group (n = 25 ) . The patients at CKD 3 -5 stages were divided into n -SHPT group (PTH ≤ 120 ng/L; n = 26) and SHPT group (PTH 〉 120 ng/L; n = 33 ) . The basic data (gender, age, body weight and blood pressure) of the patients were collected, and the serum levels of hemoglobin (Hb) , alanine aminotransferase (ALT) , glutamate aminotransferase (AST), albumin (ALB), urea nitrogen (BUN), serum creatinine (Scr), calcium, phosphorus, alkaline phosphatase (ALP), glomerular filtration rate (GFR), parathyroid hormone (FFH), soluble klotho (s-kl), 25- hydroxy vitamin D (25-OH-D), and fibroblast growth factor 23 (FGF-23) were detected. Results CKD3 -4 group and CKD5 were significantly different from CKD1 - 2 group in MAP, Hb, BUN, Scr, calcium, phosphorus, ALP and PTH ( P 〈 0. 05 ) ; CKD5 group was significantly different from CKD1 -2 in ALT (P 〈 0.05 ) ; CKD5 group was significantly different from CKD3 - 4 in Hb, ALT, AST, BUN, Scr, calcium, phosphorus, ALP and PTH (P 〈0. 05) . CKD3 -4 group and CKD5 group were significantly different from CKD1 - 2 group in the levels of s-kl, 25-OH-D and FGF-23 ( P 〈 0. 05) ; CKD5 group and CKD3 - 4 group were significantly different in the levels of s-kl, 25-OH-D and FGF-23 ( P 〈 0. 05 ) . n - SHPT group and SHPT group were significantly different in the levels of ALB, BUN, Scr, calcium, phosphorus, ALP, GFR, s-kl and 25-OH-D (P 〈 0. 05 ) . s-kl was positively correla
