摘  要:目的分析慢性乙型肝炎患者感染病原菌分布,探讨感染患者Notch通路蛋白的表达变化,为临床治疗提供参考。方法选择医院2011年5月-2014年11月200例慢性乙型肝炎感染患者(感染组)进行研究,另选择慢性乙型肝炎未感染患者180例为未感染组,150名健康体检者为对照组,采用法国生物梅里埃公司的微生物全自动鉴定仪进行病原菌鉴定分型,酶联免疫吸附测定法检测感染患者血清中Notch1、Notch2、Notch3及CAM、Bax/Bcl2的变化;研究采用WHONE5.5和SPSS20.0软件对数据进行统计分析。结果 200例慢性乙型肝炎感染患者共分离出病原菌200株,其中革兰阳性菌80株占40.0%,以肺炎链球菌22株占11.0%、酿脓链球菌15株占7.5%为主;革兰阴性菌80株占60.0%,以肺炎克雷伯菌23株占11.5%、鲍氏不动杆菌21株占10.5%为主;感染患者血清中Notch1、Notch2、Notch3及CAM、Bax水平明显高于未感染组与对照组(P<0.05),而Bcl2水平明显低于未感染组及对照组(P<0.05)。结论慢性乙型肝炎患者易感染病原菌,且感染后患者血清中Notch蛋白水平明显升高。OBJECTIVE To explore the distribution of infectious pathogens and discuss changes in expression of Notch related protein in chronic hepatitis patients so as to provide reference for clinical treatment.METHODS Totally 200 infected chronic hepatitis patients(infected group)in our hospital from May 2011 to Nov.2014 were enrolled in this study.The pathogens were identified with the full-automatic microorganism identifier(Biomerieux,Frace)and serum Notch1,Notch2,Nothch3 and CAM as well as Bax/Bcl2 were detected by EILSA after infection.Another 150 cases of healthy subjects(control group)and 180 cases of non-infected chronic hepatitis patients(non-infected group)were taken as controls.Data were statistically analyzed by software WHONE5.5and SPSS20.0provided on the bacterial resistance surveillance website of WHO.RESULTS Totally 200 pathogens were isolated from the 200 infected chronic hepatitis patients that contained 80(40.0%)gram positive strains dominated by 22(11.0%)Streptococcus pneumoniae and 15(7.5%)Streptococcus pyogenes as well as 120(80.0%)gram negative strains dominated by 23(11.5%)Klebsiella pneumoniae and 21(10.5%)Acinetobacter baumannii.The results also revealed that the levels of serum Notch1,Notch2,Nothch3,CAM and Bax of the infection group were significantly higher than those of the non-infection group and the control group(P〈0.05)while the level of Bcl2 of the infection group was significantly lower than that of the non-infection group and the control group(P〈0.05)in infected patients.CONCLUSION Chronic hepatitis patients are prone to have pathogen infections and the Notch signaling pathway is activated after infections as serum Notch protein significantly increases.
