聚维酮碘冲洗结膜囊联合抗菌药物预防白内障患者术后感染的效果研究

摘　　要：目的研究聚维酮碘稀释液冲洗结膜囊联合抗菌药物治疗对白内障患者术后感染的预防效果,为临床提供参考依据。方法选择2014年1月-2015年5月在医院行白内障囊外摘除联合人工晶体植入手术的白内障患者944例,按照随机数表法将其分为研究组和对照组,每组各472例,对照组患者术前3d采用左氧氟沙星滴眼液滴眼,研究组患者在此基础上加用聚维酮碘稀释液滴入患者结膜囊,统计两组患者眼部不良反应发生率;采集患者眼分泌物用全自动微生物鉴定分析仪鉴定培养菌种,并分析患者感染病原菌分布,比较患者干预前后病原菌阳性率。结果不良反应发生率研究组患者为0.43%,对照组患者为1.91%,差异有统计学意义(P<0.05);干预后,研究组患者细菌培养阳性率为0.21%,明显低于对照组的2.54%,差异有统计学意义(P<0.05);研究组患者分离病原菌1株,对照组患者共分离病原菌39株,两组比较差异有统计学意义(P<0.05)。结论聚维酮碘稀释液结膜囊冲洗联合抗菌药物治疗,能有效预防白内障患者术后感染及不良反应发生率,具有较好的临床使用价值。OBJECTIVE To study the effect of washing by povidone iodine diluent combined with antimicrobial agents on prevention of postoperative infections in cataract patients,and to provide the guidance for clinical treatment.METHODS Totally 944 cataract patients who received excision combined intraocular lens implantation in cataract surgery from Jan.2014 to May 2015 were selected and the patients were randomly divided into the study group and the control group,472 cases in each group.The control group was treated with levofloxacin eye drops 3days before the surgery.The patients in the study group were treated with povidone iodine diluent based on the treatment of the control group.Adverse reactions of eyes were recorded.Eye secretions of patients were sampled and analyzed by the full-automatic organism analyzer to identify cultured strains,and the distribution of pathogens was analyzed.The positive rates of pathogens before and after the intervention were compared.RESULTS The adverse reaction rate was 0.43%in the study group and 1.91%in the control group,the difference was significant（P〈0.05）.After intervention,the positive rate of bacterial culture in the study group was 1.06%,which was significantly lower than 2.54%in the control group（P〈0.05）.One strain of pathogenic bacteria was isolated in the study group and 39 strains were isolated in the control group,the difference was significant between the two groups.CONCLUSIONPovidone iodine dilute for conjunctival sac washing combined with antibacterial drugs has better clinical efficacy and can effectively reduce postoperative infections and adverse reactions in cataract patients,with good clinical practical value.
