摘　　要：目的:了解北京市三级医院医疗风险分担机制现状,分析其与医患信任的关系。方法:对北京市3家三级医院的管理者、医务人员、患者和医疗服务的其他人员,共计144人进行实地观察与个人深入访谈,在此基础上对北京市6家三级医院的593名医务人员和616名患者进行问卷调查。结果:医患双方对于医疗风险程度的评价、医疗责任保险的医疗风险分担作用和医疗风险分担意愿存在差异。结论:北京市三级医院医疗风险分担机制的不完善,对医患诊疗行为产生影响,进而影响医患信任。因此,应完善医疗风险分担的途径以促进医患信任。The objectives were to understand the current medical risk sharing mechanism in tertiary hospitals in Beijing and analysis the relationship between medical risk sharing mechanism and doctor-patient trust.The research team selected three tertiary hospitals in Beijing.Field observation and in-depth interview were used to interview hospital administrators,doctors,patients and other personnel.Total 144 people were interviewed.Questionnaire survey was used to investigate 606 medical personnel and 620 patients.The evaluation of medical risk,the function of medical liability insurance and medical risk sharing willingness were different between doctors and patients.The medical risk sharing mechanism was not enough and it had an impact on the medical diagnosis and treatment behavior,then affected doctor-patient trust.Therefore,we should improve the medical sharing mechanism to promote doctor-patient trust.
