摘 要:目的探讨高压氧联合小牛血去蛋白提取物注射液治疗高血压脑出血的临床疗效。方法选择2011年1月至2012年12月120例高血压脑出血患者,其中男性65例,女性55例;年龄31~79岁,平均年龄54.1岁。所有患者均符合高血压脑出血诊断标准,均为首次发病。随机将其分成对照组和观察组,每组患者各60例。两组患者均给予开颅手术、控制血压、降低颅内压(ICP)及高压氧等常规综合治疗,对照组患者在常规治疗的基础上给予胞二磷胆碱治疗,观察组患者在常规治疗的基础上给予小牛血去蛋白提取物注射液治疗。治疗结束后比较两组患者的临床治疗、神经功能缺损评分、血液流变学指标、ICP、格拉斯哥昏迷量表评分及不良反应情况。结果对照组患者临床治疗总有效率为63.3%,观察组患者临床治疗总有效率为83.3%;观察组患者的临床治疗总有效率明显高于对照组患者(P〈0.05)。观察组患者治疗神经功能缺损评分及格拉斯哥昏迷量表评分改善情况优于对照组患者(P〈0.05);治疗后观察组患者的全血黏度、红细胞压积和血小板聚集率均明显低于对照组(P〈0.05)。观察组患者治疗后第5天和第7天颅内压低于对照组患者(P〈0.05)。两组患者均未见肝肾功能损伤等影响治疗的严重不良反应。结论高压氧联合小牛血去蛋白提取物注射液治疗高血压脑出血临床治疗效果显著,且能够明显改善患者的神经功能,安全,副作用低,值得推广应用。Objective To investigate the effect of hyperbaric oxygen (HBO) combined with deproteinized calf blood protein extracts injection on patients with hypertensive cerebral hemorrhage. Methods From January 2011 to December 2012, a total of 120 patients with hypertensive cerebral hemorrhage were enrolled, which included 65 males and 55 females, aged 31 - 79 years old with mean age of 54.1 years old. All of them were consistence with the diagnostic criteria of hypertensive cerebral hemorrhage and first attack, which were randomly divided into control group(n = 60) and observation group(n = 60). All patients were performed by craniotomy, control of blood pressure, decreased intracranial pressure(ICP), and hyperbaric oxygen therapy. The citicoline treatment on the basis of conventional treatment was carried out in control group and the deproteinized calf bod protein extracts injection on the basis of conventional therapy was carried out in observation group. After treatment, the clinical effects, neurological function, blood theology, ICP, Glasgow coma scale score, and adverse reactions between 2 groups were compared. Results The total clinical effective rate of patients in control group was 63.3 %, and that in observation group was 83.3 %, the clinical effective rate in observation group was significantly higher than that in control group(P 〈 0.05); the improvement of neurological deficit scores and Glasgow coma scale score in observation group were better than those in control group(P 〈 0.05); after treatment, the whole blood viscosity, hematoerit, and platelet aggregation in observation group were significantly lower than those in control group(P 〈 0.05); the intracranial pressure 5-day and 7-day after treatment of observation group were lower than those of control group(P 〈 0.05); there were no serious adverse liver and kidney dysfunction in all patients.
