摘  要:目的探讨Olympus AU5421全自动生化分析仪检测血清无机磷浓度(磷钼酸紫外终点比色法)测定结果偏高中甘油三酯的作用并研究解决办法。方法在混合血清中加入不同浓度的乳糜纯品(使其浓度为349、698、1 046、2 093、4 185、6 278、7 324、8 370浊度单位),分别测定无机磷的浓度,计算出偏差,若偏差超过1/2 CLIA′88标示允许误差为有干扰;将高浓度(线性范围内)无机磷样本用0.9%氯化钠溶液(生理盐水)稀释成不同浓度分别测定3次,通过计算偏差来得到其最大与最佳稀释倍数;将无机磷浓度低值样本用0.9%氯化钠溶液稀释成4个浓度,每个浓度样本及空白样本重复测定20次,以95%下限结果大于空白样本的95%上限结果的样本浓度为检测低限。结果实验乳糜对无机磷浓度测定产生正干扰的最低浊度为349浊度单位(相当于甘油三酯浓度为1 mmol/L),无机磷浓度测定的最大稀释倍数为5倍,最佳稀释倍数为1.25倍;无机磷浓度检测低限为0.12 mmol/L。结论高甘油三酯会导致血清无机磷浓度测定结果偏高,可以通过将血清样本稀释的方法(但稀释后的浓度应大于0.12 mmol/L)加以解决。Objective To study the cause and treatment of abnormally high serum inorganic phosphorus level measured by Olympus AU5421 automatic biochemical analyzer. Methods The deviations of serum inorganic phosphorus concentration were determined by Olympus AU5421 automatic biochemical analyzer after serum mixed with different concentration of pure chylous product(concentration was 349, 698, 1 046, 2 093, 4 185, 6 278, 7 324, 8 370 turbidity unit). Concentrations of inorganic phosphorus were measured and the deviation was calculated, and presence of interference was determined with deviation beyond 1/2 CLIA'88. High concentration inorganic phosphorus samples(within linear range) were diluted with 0.9 % sodium chloride into different concentration, and the concentrations of inorganic phosphorus were measured for three times to determine the highest and best dilution fold. The low concentration inorganic phosphorus samples were diluted into 4 different concentration samples, each of them and blank sample were measured for 20 times. The low detection limit was defined as result of 95 % minimum 〉 that of 95 % maximum in blank sample. Results The positive interference of lowest turbidity chylous for inorganic phosphorus was 349 turbidity unit(equal to 1 mmol/L triglyceride). The maximum dilution fold of inorganic phosphorus for measurement was 5 times, and optimum dilution fold was 1.25 times. The lowest detection concentration of inorganic phosphorus was 0.12 mmol/L. Conclusion It is demonstrated that the high concentration of triglyeeride may induce high detection result of serum inorganic phosphorus, which can be solved by serum sample dilution(but concentration should be higher than 0.12 mmol/L).
