上海市社区青年全科医生医疗服务能力初步评估

摘　　要：目的了解上海市社区青年全科医生的医疗服务能力。方法于2014年10月至12月对本市17个区县233名25～35岁的社区全科医生的医疗服务能力进行横断面调查，以笔试和客观结构化临床考核（OSCE）相结合的方式进行评估。根据笔试及OSCE各站点得分及合格率，评估我市青年全科医生医疗服务能力。结果211名全科医生完成上述考核，平均年龄（31±2）岁，男性63名，女性148名。考核总分为（602±83）分，成绩合格率为62．6％（132／211）；其中笔试平均（64±7）分，成绩合格率为72．O％（152／211）。OSCE各站点中，项目得分最高是心肺复苏，平均（80±15）分，成绩合格率92．9％（196／211）；其次为医患沟通，平均（76±15）分，成绩合格率85．8％（181／211）。得分最低的为体格检查，平均（47±15）分，成绩合格率26．1％（55／211）；其次为心电图判读，平均（84±31）分，成绩合格率39．8％（84／211）。在基本操作中，眼底镜使用和妇科检查的得分率较低，分别为29．8％和45．4％。结论上海市社区青年全科医生具有一定的医疗服务能力，但还存在诸多不足，如体格检查、眼底镜使用和妇科检查等基本操作不规范，心电图判读能力较弱等，迫切需要加强培训。Objective To evaluate the clinical capability of young general practitioners （GPs） in communities in Shanghai. Methods A cross-sectional survey on clinical ability of young GPs was adopted from October to December 2014. Total 211 GPs aged （31 ± 2） years （25 - 35 ） from 17 districts of Shanghai participated in the study. Written examination and objective structured clinical examination （OSCE） was applied in the evaluation. Results All 211 GPs （63 male and 148 females） completed the assessment. The overall score of the assessment was （602 ± 83） and the pass rate was 62.6% （132/211）. In written examination the average score was （64±7） and the pass rate was 72. 1% （152/211）. In OSCE, the highest score （80 ± 15） and pass rate （92.9%, 196/211 ） was in CPR skill, followed by communication skill [ average score ： （76 ± 15 ） and pass rate ： 85.8 % （ 181/211 ） ]. The lowest average score was physical examination （47 ± 15） with a pass rate of 26. 1% （55/211）, followed by electrocardiogram reading [ average score ： （84± 31 ） and pass rate ： 39.8% （84/211） ]. In basic operation station, the lowest score was using funduscope and gynecologial examination （29. 8% and 45.4%, respectively）. Conclusion Young GPs in Shanghai communities basically master clinical skills, but also have some deficiencies, the training in certain skills need to be strengthened.
