北京某二级医院门诊突发性聋患者诊疗特点分析

摘　　要：目的通过分析北京市某二级医院门诊突发性聋患者的诊疗特点，为医疗资源配置提供参考。方法对2014年9月～2015年9月就诊的87例突发性聋患者的一般情况、就诊特点及治疗情况进行回顾性分析。结果①本组突聋患者主要发病年龄段为19～44岁，大专以上教育程度者较多，从事脑力劳动者较多；②耳闷／堵感、耳鸣、听力下降是突聋患者主要就诊原因，占93．1％；③从发病至就诊时间在1周内的突聋患者占77．0％；④该院门诊和住院治疗突聋的总有效率为73．5％；⑤该院初治并持续冶疗的患者32例，外院治疗该院复诊者1例，流失率为62．1％（54／87）。结论二级医院治疗突发性聋有较好的临床疗效，但复诊率低，流失率明显偏高，应充分发挥二级医院的资源优势，提高患者信任度和复诊率，进一步合理配置医疗资源。Objective To analyze the clinical characteristics of patients with sudden hearing loss from a Tier II hospital in Beijing in order to provide reference for the allocation of medical resources. Methods A questionnaire survey was conducted among 87 patients with sudden hearing loss. The general conditions, clinical characteristics and treatment outcomes were retrospectively analyzed. Results ①People of 19-44 years old, with a college education and mental jobs are the major groups with sudden deafness.②Ear fullness, tinnitus, hearing loss were three primary reasons for the patients with sudden hearing loss to visit doctors, accounting for 93.1%. ③77.0% of the patients visited the doctors within one week after the onset of hearing loss.④The total effective rate of the treatment of sudden deafness for outpatients and in-patients was 73.5%.⑤32 Patients received initial treatment at this hospital and 1 patient received treatment at other hospitals and came to this hospital for re-examination, with a loss rate of 62.1%. Conclusion The treatment outcome is good at the Tier Ⅱ hospital but the loss rate is obviously higher, so it is vital to take full advantages of the Tier Ⅱ hospital, to enhance the trust and revisit rate and to strengthen the rational allocation of medical resources.
