雷公藤多苷治疗类风湿性关节炎的临床观察及对血清VEGF、VEGFR2表达水平的影响研究

摘　　要：目的:探究雷公藤多苷治疗类风湿性关节炎(rheumatoid arthritis,RA)的临床疗效,评估其对患者血清VEGF、VEGFR2表达水平的实际影响。方法:选取自2011年12月5日至2013年12月5日来我院接受治疗的RA患者(78例),随机分为雷公藤多苷组和美洛昔康组,各39例。雷公藤多苷组给予甲氨蝶呤12mg/周+雷公藤多苷21mg 3次/d+美洛昔康7.5mg 2次/d;美洛昔康组给予甲氨蝶呤12mg/周+美洛昔康7.5mg 2次/d。比较治疗前后有效率、检测相关实验室指标,并对治疗后4、8周患者血清(VEGF)、VEGFR2水平(Elisa法)进行比较。结果:雷公藤多苷组总有效率89.7%,美洛昔康组总有效率82.1%,雷公藤多苷组在改善患者在调节免疫功能等方面优于美洛昔康组(P<0.05),两组患者实施不同治疗后4周、8周的血清(VEGF)、VEGFR2水平组间比较,雷公藤多苷组患者的指标水平明显低于美洛昔康组,差异具有显著统计学意义(P均<0.05)。结论:在临床针对RA患者实施治疗的实践过程中,采用雷公藤多苷治疗RA具有调节患者免疫功能作用,且可降低RA患者的血清VEGF、VEGFR2表达水平。Objective： To explore the the clinical curative effects of tripterygium glycosides onthe patients with rheumatoid arthritis（RA） and analysis the effect for the expression levels of serum VEGF, VEGFR2 for the patients with rheumatoid arthritis. Methods： 78 patients with rheumatoid arthritis admitted to our hospital were chosen as the research object. They were divided into the experimental group and the control group according to the random number table, and 39 cases in each group. The experimental group was taken with methotrexate 10mg / week ＋ Tripterygium glycosides 20mg tid ＋ Meloxieam 7.5mg bid, and the control group was taken with methotrexate 10mg / week 2_ Meloxicam 7. 5mg bid. The serum （VEGF）, VEGFR2 levels （Elisa method） after treatment for 4 and 8 weeks were compared. Re- suits： The totally effective rates were 89.7 % in experimental group, and 82.1% in control group~ the experimental group was superior to control group in regulating the balance of OKT4/OKT8 （P^0. 05）; after the implementation of treat- ment for 4 weeks and 8 weeks, the serum （VEGF）, VEGFR2 levels between the two groups were compared, the levels of the experimental group were significantly lower than patients in the control group, and the statistical validation data showed that differences were with statistical significance （P%0.05）. Conclusions： In clinical practice for the treatment of patients with rheumatoid arthritis, the application of tripterygium glycosides could significantly regulate the function of T lymphocyte and reduce serum VEGF and VEGFR2 expression levels in patients with rheumatoid arthritis.
