急性冠脉综合征患者冠状动脉狭窄程度与左室射血分数的相关性

摘　　要：目的探讨急性冠脉综合征(ACS)患者冠状动脉狭窄程度与左室射血分数(LVEF)的相关性。方法分析647例ACS患者,按ACS患者冠状动脉狭窄程度的Gensini评分分为2组:Gensini≥45组(328例)和Gensini<45组(319例)。比较组间基线特征差异。采用Gensini评分系统对每位患者造影结果进行评分,用以衡量冠状动脉狭窄程度。应用Spearman双侧相关分析及多因素回归分析研究Gensini评分与LVEF的关系。结果 Gensini评分较高组,其FBG、Hb A1c、WBC、LA均升高,LVEF、HDL-C及Ca2+降低。Spearman相关分析显示Gensini评分与心脏LVEF呈显著负相关(r=-0.28,P<0.01)。多因素回归分析结果表明,冠状动脉狭窄程度的积分与心脏LVEF独立相关,其偏相关系数β=-0.313,P<0.01。结论 ACS患者冠状动脉狭窄程度与LVEF独立相关。AIM To investigate the relationship between left ventricular ejection fraction （LVEF） and angiographic characteristics of acute coronary syndrome （ACS）. METHODS A total of 647 patients with ACS were divided into two groups according to Gensini score. Three hundred and twenty-eight patients were in group A with Gensini ≥45% ; 319 were in group B with Gensini 〈45%. Baseline char- acteristics were analyzed between groups. Severity of coronary atherosclerosis was defined by the Gensini score system. Statistical methods including one-way ANOVA, Kruskal-Wallis test, Spearman correlation analysis and multivariate stepwise linear regression analysis were employed to explore the relationship between EF and Gensini score. RESULTS In group A, FBG, HbAle, WBC and LA were increased. LVEF, HDL and calcium ion were decreased. Spearman correlation indicated that Gensini score signifi- cantly correlated with the LVEF （r = -0. 28, P 〈 0.01 ）. The result from the multivariate stepwise linear regression analysis showed that LVEF （ 13 = - 0. 313, P 〈 0. 01 ） was significantly and independently associated with the Gensini score. CONCLUSION EF is significantly and positively associated with Gensini score in patients with ACS.
