术前肺功能锻炼配合术后综合物理疗法对肺癌患者手术后围手术期的影响

摘　　要：目的探讨术前肺功能锻炼配合术后综合物理疗法对肺癌患者围手术期的影响。方法选择150例行肺切除的肺癌患者作为研究对象,分为给予常规护理干预的对照组(72例)和给予呼吸道干预措施的观察组(78例)。观察两组患者气管插管相关指标、手术相关指标、活动状态以及生活质量。结果观察组患者发生呼吸机相关性肺炎例数(7例)少于对照组(P<0.05)。观察组患者的平均咳嗽次数均少于对照组(均P<0.05)。观察组患者的气管插管时间、排气时间、下床时间、术后住院时间分别为(3.71±0.44)d、(1.48±0.41)d、(2.47±0.62)d和(5.95±0.75)d,均少于对照组(均P<0.05)。观察组患者的血氧饱和度[(98.28±10.25)%]、KPS评分[(85.31±8.14)分]、躯体功能评分[(72.05±7.43)分]、心理功能评分[(58.26±5.25)分]、社会功能评分[(58.37±2.43)分]、认知功能评分[(59.62±5.54)分]和总体生活质量评分[(80.64±8.27)分]均高于对照组(均P<0.05)。结论术前肺功能锻炼配合术后综合物理疗法能够缩短插管时间、改善呼吸道功能,具有积极的临床价值。Objective To explore effect of preoperative pulmonary function exercise combined with postoperative comprehensive physical therapy on patients with lung cancer after surgery perioperative. Methods Totally 150 lung cancer patients received pulmonary resection were collected and divided into the observation group（ given respiratory interventions） and the control group（ without given respiratory interventions）.Then tracheal intubation related index,operation index,activity state and quality of life were observed.Results Incidence of ventilator-associated pneumonia,intubation time,average frequency of cough,exhaust time,recovery time,postoperative hospital stay of observation group were significantly less than those in control group; Sp O2,Karnofsky performance status（ KPS） score,body function,mental function,social function,cognitive function and overall quality of life score were significantly higher than those in control group.Conclusion Preoperative pulmonary function exercise combined with postoperative comprehensive physical therapy has active clinical value for it can reduce the intubation time and improve respiratory function.
