3398例代谢综合征患者证候学分析

摘　　要：目的:研究代谢综合征(MS)的中医证候类型及分布规律。方法:按照2005年国际糖尿病联盟(IDF)关于MS的诊断标准,纳入MS患者3398例为MS组,非MS患者328例为对照组。采集一般资料、中医四诊信息和相关理化指标,分析MS中医证候分布、与相关指标间的关系。结果:常见6个证候:痰热互结证1145例(33.7%),肝火亢盛证601例(17.7%),脾虚湿盛证642例(18.9%),痰湿中阻证235例(6.9%),肝肾阴虚证543例(16.0%),兼夹血瘀证232例(6.8%)。结论:MS以痰热互结证最为多见;脾虚湿盛证女性比例大于男性,与痰热互结证相比差异有统计学意义(P<0.05);痰热互结证血压、血脂升高较明显,肝肾阴虚证血糖升高较明显。Objective： To study the TCM syndromes and distribution characteristics of metabolic syndrome（ MS）. Methods： According to the 2005 International Diabetes Federation（ IDF） criteria about MS,3398 cases with MS were included as MS group,328 cases non- MS patients as the control group,collecting general information,the four diagnostic methods information and the relevant physicochemical indexes to analyse the relationship between the distribution of TCM syndromes of MS and the related indexes. Results： There were 6 main syndromes,phlegm heat stasis syndrome included 1145 cases（ 33. 7%）,hyperactivity of liver fire excess syndrome 601 cases（ 17. 7%）,spleen dampness syndrome 642 cases（ 18. 9%）,syndrome of stagnation of phlegm dampness 235 cases（ 6. 9%）,syndrome of liver and kidney Yin deficiency 543 cases（ 16. 0%）,accompanied with blood stasis syndrome 232 cases（ 6. 8%）. Results： Conclusion： The most common syndrome of MS was phlegm heat stasis syndrome. The percentage of female with spleen dampness syndrome was higher than that of male. Compared with phlegm heat stasis syndrome,the difference was statistically significant（ P〈0. 05）. Blood pressure and blood lipids were significantly elevated in phlegm heat stasis syndrome while in syndrome of liver and kidney Yin deficiency,blood glucose increased obviously.
