临床药师参与临时进药管理实践研究

摘　　要：目的规范医院临时进药行为。方法通过临床药师参与临时进药管理,建立有临床药师参与的规范化制度和流程,并设计相关软件来规范临时进药行为。结果 2014年临时进药申请数共643次,超适应证申请占55.68%,抗菌药物申请占12.65%;不合格退回申请数74次,占总申请的11.51%,其中申请理由不充分占54.05%,填写不规范占44.56%。结论临床药师参与临时进药管理,能进一步规范临时进药行为,同时为临床药师参与临床提供平台。Objective To standardize the hospital＇s temporary drug purchase management.M ethods Through the participation of clinical pharmacists in temporary drug purchase management,the standardization of systems and processes was established,and the related software to regulate the behavior of temporary drug purchase was designed.Results In 2014,the total applications of tern-porary drug purchase was 643 times.The off- label use applications ratio was 55.68%,and the antibacterial drug application ratio accounted for 12.65%;the total unqualified applications was 74 times,accounting for 11.51%,among which the ones not fully reasonable accounting for 54.05%,the ones not standardized written accounting for 44.56%.Conclusion Clinical pharmacists participating in temporary drug purchase management can standardize the behavior of temporary drug purchase,and can offer a platform for clinical pharmacists in participating in clinical.
