摘  要:目的探讨超声弹性成像应变率比值法对甲状腺微小癌的诊断价值.方法对68例甲状腺疾病患者83个病灶行非探头加压式超声弹性成像检查,计算正常腺体组织与病变区组织的弹性应变率比值(SR),以病理结果为诊断标准,绘制ROC曲线并确定鉴别良恶性病灶的最佳诊断界点.结果 83个病灶中经病理诊断恶性病灶34个,良性病灶49个;恶性病灶SR为3.886±0.572,良性病灶SR为1.759±0.629,两者差异具有统计学意义(P<0.05).ROC曲线下面积为0.938,以3.26为最佳诊断界点时,诊断的敏感性为85.3%,特异性为91.8%,准确率89.2%.结论超声弹性成像对甲状腺微小癌的诊断具有重要应用价值.Objective To assess the value of ultrasonic elastography strain ratio method in diagnosing thyroid microcarcinoma. Method Sixty-eight patients with eighty-three masses were examined by UE strain ratio method( without pressure) to calculate the stain ratio( SR) of normal glandular tissue and lesions,and confirmed by pathology. ROC curve was used to determine the cut-off point of SR. Results There were 34 malignant lesions and 49 benign lesions according to the pathology. The difference on the mean SR between malignant lesions( 3.886 ± 0. 572) and benign lesions( 1. 759 ± 0. 629) was statistically significant( P 0. 05). The area under the ROC curve was 0. 938. While taking 3. 26 as the cut-off point of SR,the sensitivity was 85. 3%,and the specificity was 91. 8%,and the accuracy was 89. 2%. Conclusion Ultrasound elastography has significant value on the diagnosis of thyroid microcarcinoma.
