异丙酚复合瑞芬太尼应用于老年高血压患者手术麻醉中的效果研究

摘　　要：目的研究异丙酚复合瑞芬太尼应用于老年高血压患者手术麻醉中的效果。方法选取我院自2013年11月~2015年7月收诊的老年高血压患者80例,采取数字随机法分成观察组（n=40）和对照组（n=40）;对照组采取异丙酚麻醉,观察组则采取异丙酚复合瑞芬太尼麻醉,比较两组的麻醉效果。结果观察组T1时DBP高于对照组,差异有统计学意义（P〈0.05）;观察组T1时SBP、DBP均低于T0时,差异有统计学意义（P〈0.05）;对照组T1时SBP、DBP、HR均低于T0时,差异有统计学意义（P〈0.05）;对照组T2时SBP低于T0时,差异有统计学意义（P〈0.05）;观察组硝酸甘油及艾司洛尔的用量低于对照组,差异有统计学意义（P〈0.05）;观察组OAA/S评分高于对照组,差异有统计学意义（P〈0.05）;观察组拔管时间长于对照组,差异有统计学意义（P〈0.05）;观察组并发症发生率5.00%,对照组并发症发生率25.00%,观察组并发症发生率低于对照组,差异有统计学意义（P〈0.05）。结论异丙酚复合瑞芬太尼应用于老年高血压患者手术麻醉中的效果显著,并且可以维持血压以及血流动力学稳定,用药安全性也较高,具有很高的临床价值。Objectives To investigate the effects of propofol combined with remifentanil applied in surgical anesthesia in elderly patients with hypertension. Methods A total of 80 elderly patients with hypertension hospitalized in our hospital from November 2013 to July 2015 were selected and randomly divided into the observation group（n=40） and the control group（n =40）. Patients in the control group were anesthetized by propofol, while patients in the observation group were anesthetized by propofol combined with remifentanil. The anesthetic effects of the two groups were compared. Results DBP at T1 was significantly higher in the observation group than the control group（P〈0.05）. In the observation group, SBP and DBP were significantly lower at T1 than T0（P〈0.05）. In the control group, SBP, DBP, and HR were significantly lower at T1 than T0（P〈0.05）, and SBP was significantly lower at T2 than T0（P〈0.05）. The dose of glyceryl trinitrate, esmolol used in the observation group was lower than that in the control group with statistically significant difference（P 0.05）. The OAA/S score in the observation group was higher than that in the control group with statistically significant difference（P〈0.05）. The extubation time in the observation group was longer than that in the control group with statistically significant difference（P〈0.05）. The incidence of complications in the observation group was 5.00%, which was significantly lower than 25.00% in the control group（P〈0.05）. Conclusion The effect of propofol combined with remifentanil is significant when applied in surgical anesthesia in elderly patients with hypertension,which can stabilize blood pressure and hemodynamics with high safety and clinical value.
