摘  要:目的观察慢性重型肝炎伴侵袭性肺真菌感染患者螺旋CT中的特征,为临床提高慢性重型肝炎伴侵袭性肺真菌感染诊断的准确率提供依据。方法选取2011年1月-2014年12月在医院进行诊治的76例慢性重型肝炎伴侵袭性肺真菌感染患者进行螺旋CT扫描观察,观察其CT特征。结果 76例慢性重型肝炎伴侵袭性肺真菌感染患者中感染假丝酵母属31株、曲霉菌属19株、新型隐球酵母菌13株、组织胞浆菌11株、肺孢子菌2株;CT特征为结节、实变、网格或线样影、磨玻璃影以及胸腔积液的分布率,分别为38.2%、34.2%、14.5%、7.9%、5.3%;慢性重型肝炎伴侵袭性肺真菌感染在CT扫描中具有一致性。结论螺旋CT在慢性重型肝炎伴侵袭性肺真菌感染中能起到较好的诊断与辅助作用,能提高临床诊断率,并其图像学资料能作为治疗的依据。OBJECTIVE To investigate the spiral CT features of chronic severe hepatitis with invasive pulmonary fungal infections so as to provide the basis for improvement of the clinical diagnostic accuracy of chronic severe hepatitis with invasive pulmonary fungal infections.METHODS Totally 76 cases of chronic severe hepatitis with invasive pulmonary fungal infections during Jan.2011 to Dec.2014 were selected and observed by spiral CT scan.RESULTS Among the 76 cases of chronic severe hepatitis with invasive pulmonary fungal infections,31 cases were infected by Candida,19 by Aspergillus,13 by Cryptococcus neoformans,11 by Histoplasma and2 by Pneumocystis.CT was characterized by nodules,consolidation,grid or wire-like shadow,ground-glass opacities and pleural effusion,accounting for 36.8%,34.2%,15.8%,7.9%,5.3%,respectively.Descriptions of chronic severe hepatitis with invasive pulmonary fungal infections had consistency in CT scan.CONCLUSIONSpiral CT in chronic severe hepatitis with invasive pulmonary fungal infections can play a better role in the diagnosis and auxiliary diagnosis and improve the clinical diagnosis rate,and the imaging data can be used as the basis of treatment.
