改良式体位引流在消化外科肺部感染患者中的应用研究

摘　　要：目的探讨改良式体位引流在消化外科肺部感染患者中的应用效果,为临床护理提供参考依据。方法将2013年5月-2015年5月医院消化外科92例肺部感染患者按随机数字表法分为对照组46例和研究组46例,对照组患者给予常规治疗和护理,研究组患者在对照组基础上采用改良式体位引流,比较两组患者治疗时间、恢复状况及肺部感染各项指标的差异,数据采用SPSS17.0软件进行统计分析。结果患者治疗10d后,研究组白细胞计数及C-反应蛋白恢复天数、发热天数均较对照组明显减少,差异有统计学意义(P<0.01);研究组患者发生肺不张4例,发生率为8.7%;对照组患者发生肺不张17例,发生率为37.0%;研究组患者肺不张发生率低于对照组,差异有统计学意义(P<0.01);研究组患者疗效显著高于对照组,差异有统计学意义(P<0.01)。结论改良式体位引流可以促进消化外科肺部感染患者恢复,疗效优于传统的体位引流,值得临床推广。OBJECTIVE To investigate the effect of modified postural drainage on digestive surgical patients with lung infections,so as to provide reference for clinical nursing.METHODS Totally 92 patients with lung infection in the digestive surgery department from May 2013 to May 2015 were selected and divided into the control group（n= 46）and the study group（n=46）.The control group was given conventional treatment and care,and the study group was given modified postural drainage on the basis of care in the control group.The treatment duration,recovery and the indicators of lung infection were compared between the two groups.RESULTS After treatment for10 days,the time of white blood cell count and CRP to recover and the duration of fever were significantly reduced in the study group compared with the control group,the difference was significant（P〈0.01）.Pulmonary atelectasis occurred in 4cases in the study group with the incidence of 8.7% and in 17 cases in the control group with the incidence of 37.0%,indicating the incidence of pulmonary atelectasis was significantly lower in the study group than in the control group（P〈0.01）.The efficacy was higher in the study group than in the control group,with statistical significance（P〈0.01）.CONCLUSION Modified postural drainage can contribute to recovery of indigestive surgical patients with lung infections and is more effective than traditional postural drainage.Modified postural drainage is worthy of promotion.
