肾上腺B超筛查在新生儿重度高胆红素血症中的应用

摘　　要：目的探讨肾上腺B超在新生儿重度高胆红素血症中的筛查应用，为早期发现肾上腺出血提供依据。方法对2011年7月1日-2014年8月1日入住新生儿科的重度高胆红素血症[血清总胆红素（TSB）〉342μmol／L]的新生儿行肾上腺B超筛查。结果入选的176例重度高胆红素血症的新生儿中有肾上腺出血的有12例，12例均为足月顺产儿，4例为巨大儿，2例有产钳娩出史。合并颅内出血的有3例，治疗过程中有2例合并发热。结论肾上腺B超筛查在重度高胆红素血症中应用可以早期发现肾上腺出血。Objective To evaluate the adrenal ultrasound screening application in neonatal severe high blood bilirubin for the early detection of adrenal hemorrhage. Methods From July 1, 2011 to August 1, 2014, in our hospital the newborns of severe high blood bilimbin （TSB 〉 342μ mol/L） were screened by the neonatal adrenal ultrasound. Results Neonatal adrenal hemorrhage were 12 cases of 176 cases of severe high blood bilirubin, and all 12 cases were full-term births, 4 cases of macrosomia, 2 cases with a history of forceps childbirth. Conclusion Adrenal ultrasound screening can be applied in the severe high blood bilirubin for early detection of adrenal hemorrhage.
