摘 要:目的探讨宝石CT低剂量兔颅脑灌注的可行性。方法健康新西兰大白兔20只,每只分别用固定管电压80kV及不同管电流80、50、10mA行颅脑CT灌注扫描。采用自适应性统计迭代算法(ASIR),比较三组方案的有效剂量(E)、灌注参数脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)值及功能图信噪比(SNR),并主观评价图像质量。结果三组的有效剂量分别为1.39、0.87和0.17mSv,50mA组较80mA组减少37.41%,10mA组较80mA组减少87.77%。三组灌注参数CBF、CBV、MTT均值相比,80mA与50mA组间差异无统计学意义,80mA与10mA组、50mA与10mA组间差异有统计学意义。三组测量的sNR相比,80mA与50mA组间差异无统计学意义,两组分别与10mA组间差异有统计学意义。主观评价图像质量,差异无统计学意义。结论从实验可知,宝石CT兔颅脑灌注应用ASIR^JI权算法,可明显降低灌注剂量,即低剂量灌注,并且图像质量无明显影响。因此,为探讨低剂量人脑灌注提供可行性参考。Objective To determine the feasibility of low dose cerebral CT perfusion protocol with a rabbit model. Methods 20 New Zealand rabbits received cerebral CT perfusion imaging using a tube voltage of 80 kV with tube current of 80, 50 and 10 mA. Images were reconstructed with ASIR. To compare the effective dose and parameter of CBF, CBV, MTT maps and SNR were generated from the CT perfusion-4 software (ADW 4.6) and evaluate the image quality subjectively. Result The effective dose values were 1.39, 0.87 and 0.17 mSv for the 80, 50 and 10 mA groups, respectively. There was 37.41% dose reduction for the 50 mA group compared with the 80 mA group and 87.77% dose reduction for the 10 mA with 80 mA group. For average value of CBF, CBV and MTT maps, there was no statistically significant difference for 80 mA and 50 mA. There was statistically significant difference for 80 mA and 10 mA, 50 mA and 10 mA (P 〉 0.05). Similar results were obtained for SNR. Subjective image quality scores of 80 mA and 10 mA groups had no significant difference, the same as that of 50 mA group (all P 〉 0.05). Conclusion Low dose perfusion scan with ASIR reconstruction produces similar perfusion map and parameters as high dose scans with a rabbit model with more dose reduction: low-dose perfusion may serve as reference for low-dose CT perfusion of the human brain perfusion.
