摘　　要：目的:对比腹腔镜手术和开腹手术治疗新生儿腹部囊肿的效果。方法:选择因腹部囊肿就诊于南京医科大学附属南京儿童医院新生儿外科的84例新生儿,将患儿分为腹腔镜手术组(40例)和开腹手术组(44例),对两组患儿的手术时间、术后进食时间、住院时间和术后并发症等临床指标进行对比分析。结果:卵巢囊肿、网膜囊肿腹腔镜手术组在手术时间上显著低于开腹手术组,而在术后进食时间、住院时间上,腹腔镜手术组与开腹手术组没有差异。肠重复畸形、肠系膜囊肿在手术时间、术后进食时间、住院时间腹腔镜手术组较开腹手术组短,且腹腔镜手术组手术切口相对较小。开腹手术组的术后并发症包括伤口感染2例和肠粘连2例,腹腔镜手术组没有术后切口感染和肠粘连病例发生。结论:腹腔镜手术治疗新生儿期腹部囊肿是安全可靠的,疗效满意,切口美观。Objective： To compare the outcomes of open surgery and laparoscopy assisted surgery on neonatal abdominal cystic masses. Methods： This was a retrospective review of 84 patients with abdominal cystic masses who were treated in the Department of Neonatal Surgery at Nanjing Children＇s Hospital affiliated to Nanjing Medical University. The patients were divided into laparoscopic-assisted surgery and laparotomy groups. Differences in operating time,post-operative fasting time,post-operative hospital stay,and post-operative complications were reviewed. Results： The surgical time in the laparoscopy assisted surgery for ovarian cyst and omental cyst was significantly shorter than that in the laparotomy group,and there was no statistical difference of post-operative fasting time and post-operative hospital stay between the two groups. For intestinal duplication and mesenteric cyst,surgical time,post-operative fasting time and post-operative hospital stay were shorter in laparoscopic-assisted surgery group than in laparotomy group. The incision in laparoscopy assisted surgery group was smaller than laparotomy groups. The post-operative complications in laparotomy group included wound infections in 2 cases and adhesions in 2 cases,but no post-operative complications occurred in the laparoscopy assisted surgery group.Conclusion： Laparoscopy assisted surgery for abdominal cystic masses in neonates is a safe and reliable procedure with good cosmetic outcomes.
