狗皮膏(改进型)缓解椎管内麻醉术引起腰部不适的临床效果观察

摘　　要：目的：观察狗皮膏（改进型）外敷缓解椎管内麻醉术后腰部不适的临床效果。方法：2014年3~6月我科椎管内麻醉术后出现腰部不适患者100例,采用随机数字表法分为实验组和对照组各50例,实验组患者给予常规的治疗及心理护理,同时在腰部不适处贴敷狗皮膏（改进型）,根据病人的缓解程度24 h后给予更换。对照组患者采取常规的治疗及心理护理。根据两组患者在主诉腰部不适时及其后6、12、24、48 h进行的疼痛NRS评分、缓解程度情况及不良反应比较评估两组治疗效果。结果：实验组和对照组NRS评分在发生腰部不适6、12、24、48 h比较,差异具有统计学意义（P〈0.05）;实验组发生腰部不适48 h缓解49例,总缓解率98%;对照组发生腰部不适48 h缓解40例,总缓解率80%;两组在治疗期间均未出现不良反应。结论：狗皮膏（改进型）外敷缓解椎管内麻醉术后腰部不适效果明显,值得临床推广。Objective： To observe the clinical effect of Gou-pi plaster （improved） to alleviate postoperative lumbar discomfort after intraspinal anesthesia. Methods： From March 2014 to June 2014, 100 cases with postoperative lumbar discomfort after intraspinal anesthesia from our department were included and dived into experimental group （ n = 50, receiving common treatment, psychological nursing, and Goupi plaster （improved） ） and control group （n = 50, receiving common treatment and psychological nursing ） by random digital table method. The Gou-pi plasters （improved） were replaced after 24 h according to the discomfort of the patients. The therapeutic effects were evaluated by NRS pain score, alleviation degree and side effects at different time points after the patients complained pain（6, 12, 24, 48 h）. Results： NRS pain scores were of statistical significances （P 〈0.05） at time points of 6, 12, 24, 48 h. After 48 h, 49 patients in experimental group experienced the alleviation of lumbar discomfort, while only 40 patients in control group experienced the alleviation. No side effects were observed in both two groups. Conclusion： Gou-pi plaster （improved） can effectively alleviate postoperative lumbar discomfort after intraspinal anesthesia, which is worthy of promotion.
