摘 要:目的:通过研究966例小儿肺炎的病原学情况,了解镇江地区儿童肺炎8种病原体感染流行情况,为临床提供科学的诊断依据。方法:采用间接免疫荧光法检测2014年1月至2014年12月于江苏大学附属医院儿科住院的966例肺炎患儿血清腺病毒、呼吸道合胞病毒、甲型流感病毒、乙型流感病毒、副流感病毒及肺炎支原体等8种呼吸道病原体免疫球蛋白M。结果:966例肺炎患儿中甲型流感病毒阳性率最高,检出阳性病例215例,总阳性率为22.26%。其次为乙型流感病毒,检出阳性病例89例,总阳性率为9.21%,秋冬季阳性率最高,春夏最低,1~3岁患儿感染率最高,6个月以内最低。支原体阳性病例53例,总阳性率为5.46%,明显低于其他地区的发病率,秋季阳性率最高,春季最低。混合阳性为54例,其中2种混合感染为45例,3种为9例,占总病例数的5.6%。结论:甲型流感病毒、乙型流感病毒及支原体是2014年镇江地区儿童肺炎的主要病原,甲型流感病毒呈全年流行趋势,秋冬更甚,其中6个月~14岁感染较多,1~3岁的婴幼儿高发;乙型流感病毒以秋冬季流行为主,1~3岁小儿感染高发,且出现以1~3岁及3岁以上学龄前儿童的较多混合感染,应高度重视、科学诊疗。Objective: To investigate the pathogen distribution characteristics of 966 children with pneumonia in Zhenjiang area. Methods: The 966 children who were hospitalized with pneumonia in the pediatric ward of affiliated hospital of Jiangsu university from January 2014 to December 2014 were enrolled. Serum specimens were collected and tested by indirect immunofluorescence for adenovirus, respiratory syncytial virus, influenza A virus (IAV), influenza B virus(IBV), parainfluenza 1, 2, 3, mycoplasma pneumonia (MP), chlamydia pneumonia, and Legionella pneumophila. Results: Among the eight pathogens, the predominant pathogen was with outbreak peak of autumn and winter, and the group with the highest positive rate was 1-3 years ,the lowest was 0-6 months. The total positive rate of IAV was 22. 26% ,the total positive rate of IBV was 9.21% ,the season with the highest positive rate was autumn,the lowest in summer. And for MP, the total positive rate was 5.46%, the season with the highest positive rate was autumn, the lowest in spring, 54 cases with mixed infection were found, among these 54 cases, the total positive rate was 5.6% , 45 cases with two pathogens were detected, three pathogens were 9 cases. Conclusion: IAV, IBV and MP from January 2014 to December 2014 are the main pathogens in children with pneumonia in Zhenjiang area, distribution difference in age and season, mixed infections are frequently detected.
