认知行为干预对产妇产后抑郁影响的临床研究

摘　　要：目的探讨认知行为干预对产妇产后抑郁的临床应用效果。方法将290例初产妇分为试验组150例和对照组140例,对照组给予常规治疗和护理,试验组在常规护理的基础上采取认知行为干预护理,从产妇心理角度、行为角度等实施认知行为干预。比较两组产妇出院时及出院后6个月抑郁状况和血清肾上腺素、去甲肾上腺素水平。结果出院前两组产妇爱丁堡产后抑郁量表评分得分和血清肾上腺素、去甲肾上腺素水平差异无统计学意义(P>0.05);出院6个月试验组产妇爱丁堡产后抑郁量表得分和血清肾上腺素、去甲肾上腺素水平均低于对照组(P<0.05);试验组产妇的产后抑郁发生率低于对照组(P<0.05)。结论认知行为干预对预防和消除产后抑郁有明显效果。Objective To investigate the clinical effect of cognitive behavioral intervention on postpartum depression. Methods A total of 290 patients with the initial points for the test group 150 cases and the control group 140 cases,the control group was given conventional treatment and nursing,the experimen- tal group on the basis of routine nursing care to take cognitive behavioral nursing intervention, from maternal psychological angle, angle and the maternal behavior around the joint relationship perspective of cognitive behavior intervention. A comparison of 6 groups of patients discharged from hospital and two months after the Edinburgh postnatal depression scale score and serum epinephrine, norepinephrine levels. Results The discharge of two groups of Edinburgh Postnatal Depres- sion Scale table score and serum levels of adrenaline and to the armor adrenalin level no difference （ P 〉 0.05 ） , discharge 6 months in the experiment group, the Edinburgh Postpartum Depression volume table score and serum levels of epinephrine and norepinephrlne levels were lower than those in the control group （ P 〈 0.05 ）. Test groups of maternal postpartum depression occurrence rate is lower than the control group （ P 〈 0.05 ）. Conclusion The effect of cognitive behavior intervention on the prevention and elimination of postpartum depression has obvious effect.
