中药煮散、蜜丸联合抗病毒药物治疗乙肝肝硬化腹水

摘　　要：目的使用传统剂型中药煮散、蜜丸剂联合抗病毒药物治疗乙型病毒性肝炎后肝硬化腹水患者,观察其临床疗效。方法将我院80例乙型病毒性肝炎后肝硬化腹水的患者随机分为治疗组、对照组各40例,两组患者基础治疗均使用拉米夫定片联合阿德福韦酯片口服抗病毒,及常规西药,对照组基础治疗上加服益肝灵分散片口服治疗;治疗组在基础治疗上,有腹水时加用中药辩证加减,以煮散形式口服,无腹水时根据中医辩证分型以大蜜丸形式口服,疗程均为6个月,6个月后仅维持基础治疗。观察2组患者治疗前、治疗6个月后、12个月后的患者症状、肝功能、肝脾B超及腹水、凝血酶原时间、HBV-DNA等各项指标。结果治疗6个月后,治疗组患者症状、腹水消退情况、肝功能结果等综合疗效优于对照组;治疗12个月后评价患者治疗前后肝功能,采用Child-Pugh评分,治疗组总有效率及临床缓解率均高于对照组。结论传统剂型中药煮散剂、蜜丸剂可适用于临床治疗乙型病毒性肝炎后肝硬化腹水,值得推广。Objective To observe the clinical effect of Chinese medicine powder for boiling and honeyed pill combined with antiviral medicine in the treatment of hepatitis B liver cirrhosis ascites. Methods 80 cases of hepatitis B with cirrhosis ascites patients in our hospital were randomly divided into treatment group and control group, and each group had 40 cases. They were given basic treatment of lamivudine combined with adefovir dipivoxil tablet antiretroviral therapy, and conventional Western medicine. The control group took oral-ly additionally Yiganling tablet. The treatment group was given additionally Chinese medicine powder for boiling when ascites, otherwise took orally additionally big honeyed pill. The course of treatment was 6 months, and then maintains the basic treatment only after 6months. The indicators symptoms, liver function, liver and spleen B ultrasonic and ascites, prothrombin time, HBV-DNA before and afterment treatment, 6 months of treatment and 12 months of treatment were observed. Results After 6 months of treatment, symptom, ascites and liver function of the treatment group were better than those of the control group, etc. After 12 months of treatment, the evaluation of liver function before and after treatment, using the Child-Pugh score, the total effective rate and clinical remission rates were higher of the treatment group were higher than those of the control group. Conclusion The traditional Chinese medicine powder for boiling and hon-eyed pill is suitable for clinical treatment after hepatitis B cirrhosis ascites, and is worth promoting.
