原发性干燥综合征合并周围神经病变患者的血清免疫学变化

摘　　要：目的分析原发性干燥综合征(primary Sjgren's syndrome,PSS)合并周围神经病变患者的血清免疫学改变。方法选择2011—2014年本院收治的PSS患者86例,依据患者是否伴有周围神经病变分为2组,观察组25例:合并周围神经病变;对照组61例:单纯PSS。分析两组患者血清免疫学指标。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果原发性干燥综合征累及周围神经的比例为29.1%(25/86)。观察组血清TNF-α水平为(31.91±7.73)μg/L,显著高于对照组的(25.26±5.32)μg/L,差异有统计学意义(t=4.586,P<0.05)。观察组患者为20.0%(5/25)显著高于对照组的1.6%(1/61),差异有统计学意义(P<0.05)。结论PSS患者有一定比例累及周围神经,合并周围神经病变的患者血清TNF-α水平及SSA、SSB双阳性比例增高。Objective To analyze the serum immunological changes of primary Sjgren syndrome（ PSS） combined with peripneral neurepathy in patiehts. Methods Clinical data of patients with PSS received treatment from 2011 to 2014 was retrospectively analyzed. Patients were divided into two groups,observation group： PSS with peripheral neuropathy,control group： PSS without peripheral neuropathy. Measurement data was processed by t test,count data was used by chi square test,P〈0. 05 was considered statistically significant. Results The rate of PSS with peripheral neuropathy was 29. 1%（ 25 /86）. Patients of observation group had a higher level of TNF- α than that of control group,the difference was statistically significant（ t = 4. 586,P〈0. 05）. However,observation group had a higher rate of double positive rate of SSA and SSB than that of control group,the difference was statistically significant（ χ^2= 9. 211,P〈0. 05）. Conclusions Peripheral neuropathy is not a rare manifestation of PSS,and PSS patients of PN have a high levels of TNF- α and a high rate of double positive rate of SSA and SSB.
