糖尿病合并上消化出血的临床分析和治疗

摘　　要：目的对糖尿病合并上消化出血的临床治疗方案进行分析。方法将80例糖尿病合并上消化出血的患者按数字随机表法分为观察组和对照组,每组各40例,80例患者均于2013年4月—2014年8月期间在该院接受治疗。两组患者入院后均予以对症止血措施,其中观察组40例患者静脉滴注葡萄糖时加入胰岛素,另于每日清晨8时皮下注射甘精胰岛素;对照组40例患者静脉滴注胰岛素治疗。对比观察两组方案的治疗效果。结果 1观察组患者治疗后平均血糖明显低于对照组,且无患者发生低血糖,组间差异具有统计学意义,P<0.05。观察组普通胰岛素用量明显少于对照组,大便潜血转阴时间明显短于对照组,P<0.05。2观察组患者的治疗总有效率为92.5%,比对照组高17.5%,组间差异具有统计学意义,P<0.05。结论在常规治疗的基础上加用甘精胰岛素治疗糖尿病合并上消化出血的临床效果显著,可有效控制患者血糖,改善其临床症状,从而提高患者的生活质量。Objective to analyze the clinical treatment of diabetic patients with upper gastrointestinal hemorrhage. Methods80 cases of diabetic patients with upper gastrointestinal bleeding were randomly divided into observation group and control group, 40 cases in each group, 80 patients were treated in our hospital from August 2014 to April 2013. The two groups of patients after admission to the right to stop bleeding, In the observation group, 40 patients were treated with insulin, and insulin was injected subcutaneously at 8 hours per day; Control group of 40 patients with intravenous infusion of insulin therapy. To observe the therapeutic effect of the two groups. Results 1 the average blood glucose level in the observation group was significantly lower than that in the control group, and no patients had hypoglycemia, the difference between the groups was obvious,P〈0.05. The observation group average dose of insulin was significantly less than the control group, fecal occult blood clearance time was shorter than the control group, P 〈0.05. 2 the total effective rate of the observation group was 92.5%, higher than that of the control group, the difference was significant between the groups, P〈0.05. Conclusion on the basis of conventional treatment, the clinical effect of treatment of diabetic patients with upper digestive hemorrhage with insulin and insulin therapy is significant, which can effectively control the blood glucose and improve the clinical symptoms,so as to improve the quality of life of patients.
