摘  要:目的探讨颅内压(ICP)监测在自发性脑出血术后患者治疗中的临床意义。方法应用ICP监测技术对23例自发性脑出血患者术后进行监测,根据术后监测ICP值波动范围,指导临床脱水药物应用或及时再次手术治疗。结果本组患者监测ICP 24 h,每2 h 1次,根据ICP监测平均值,术后6 h ICP出现峰值。术后1个月患者日常生活能力(ADL)分级法评定,Ⅰ级(完全恢复日常生活)1例,Ⅱ~Ⅲ级(部分恢复自理能力,需人帮助,轻残)15例,Ⅳ~Ⅴ级(卧床或植物生存状态,重残)3例,死亡或自动出院4例。结论自发性脑出血患者术后早期应用ICP监测有助于及时发现病情变化及指导治疗,降低并发症,提高疗效和判断预后。Objective To explore the clinical effects of intracranial pressure (ICP) monitoring for the patients with spontaneous intracranial hemorrhage after operation. Methods ICP monitoring was conducted in 23 cases of the patients with spontaneous intracranial hemorrhage after operation, we assessed the therapeutic effects and prognosis of ICP monitoring in the patients with spontaneous intracranial hemorrhage after operation. Dosage of dehydrater were adjusted by ICP value. Results ICP monitoring was conducted every 2 hours during 24 hours, and the peak of the ICP monitoring was appeared after 6 hours post-operation. ADL of all the patients were determined after one month. 1 case was in good prognosis, 15 in moderate disability,3 in severe disability and 4 died. Conclusion ICP monitoring could offer available parameters, let doctors know intracranial pathologic changes, and make exact early therapeutic regimen, reduce complications, improve the curative effects and evaluate prognosis of the patients with spontaneous intracranial hemorrhage after operation.
