摘　　要：目的:探讨精神病专科医院临床药师对精神障碍患者实施药学监护的必要性。方法:结合某精神病专科医院2014年所有出院患者的医嘱点评结果,对其中43份不合理用药医嘱进行分析。结果:不合理用药医嘱涉及偏执型精神分裂症、焦虑障碍、癫痫等患者。临床药师可通过查阅患者的医嘱用药信息,依据药动学和药效学在药物品种选择、给药途径、给药方法、给药剂量和疗程等方面给予临床医师建议,同时可对患者的用药过程进行监测,发现、防止和解决与用药有关的问题,保障患者安全、有效、经济用药。结论:应积极倡导"以患者为中心"的药学服务理念,深入推进精神病专科医院临床药师制建设,临床药师有必要深入临床开展药学监护工作。OBJECTIVE：To probe into the necessity of implementation of pharmaceutical care in patients with mental disorders by the clinical pharmacists in the psychiatric hospital.METHODS：43 cases of irrational medical orders were analyzed combined with the medical order reviews from all discharged patients in a certain psychiatric hospital in 2014.RESULTS：The irrational medical orders involved patients like paranoia schizophrenia,anxious disorder and epilepsy,etc.Through reference of the medication information of patients and suggestions that given to the clinician based on the selection of drug categories,route of administration,method of administration,dosage of administration and treatment course,the clinical pharmacists could monitor the administration process of patients and then find out,prevent and solve the related problems,so as to guarantee the safe,effective and economic medication in patients.CONCLUSIONS：It is necessary to actively advocate the pharmaceutical service concept of＂taking patients as the center＂,further contribute the construction of clinical pharmacists system in the psychiatric hospital,and the clinical pharmacists should develop pharmaceutical care in in-depth clinic.
