摘  要:目的研究64层螺旋CT血管造影(CTA)对婴儿法洛四联症合并肺动脉闭锁(To F-PA)和主要体肺侧支血管(MAPCAs)的诊断准确性。方法回顾分析18例法洛四联症合并To F-PA和MAPCAs的婴儿患者,其中男性11例,女性7例,年龄1~334 d,平均年龄6 d。通过CTA分析MAPCAs数量、直径、起源和所供应的肺叶以及中心肺动脉存在与否和它们的直径,运用斯皮尔曼等级相关系数计算CTA与DSA(n=16)或手术证实(n=2)所测量的管径数据之间的相关性。结果发现在所有病例中CTA与DSA或术中发现的MAPCAs的数量、直径、起源和所供应的肺叶有着一致性,在18例中的11例患者中,CTA准确显示中心肺动脉。同时我们发现CTA和DSA对MAPCAs(r=0.83)和中心肺动脉(r=0.82)的管径测量值上有着比较好的相关性。结论 CTA能够有效显示侧支循环血管及中心肺动脉,能够明确体肺动脉侧支血管的数量、起源、走行、直径和所供应的肺叶。Objective To study with 64-slice spiral CT Angiography(CTA) for infants with tetralogy of fallot with pulmonary atresia(To F-PA) and leading pulmonary collateral vessels(MAPCAs) diagnostic accuracy. Methods 18 cases of tetralogy of fallot with To F-PA and MAPCAs infants were retrospective analysed, including 11 males, 7 females, mean age of 6 days, range 1~334 days. By CTA origin analysis of a MAPCAs number, diameter, and the supply of lungs as well as whether it has centers pulmonary and their diameter, using the Spearman rank correlation coefficient of CTA and DSA(n=16) or surgery(n=2) the diametermeasurement by correlation of data. Results In the case of all CTA and DSA or MAPCAs found in the number of origin, diameter, and the lobe has a consistency of supply, 11 patients in the 18 cases, CTA display centers pulmonary hypertension. And we find that CTA and DSA MAPCAs(r=0.83) and central pulmonary arterial hypertension(r=0.82) diameter there is good correlation between the measured values. Conclusion The CTA can effectively display centre for vascular and pulmonary collateral circulation, to clear pulmonary collateral vessels of the quantity, origin, course, diameter and the supply of lungs.
