摘  要:目的研究基于镜像疗法(MT)结合肌电生物反馈(EMGBF)对脑卒中患者手功能的治疗效果。方法脑卒中偏瘫患者60例,随机分为对照组和治疗组各30例。两组患者均接受常规康复训练及EMGBF治疗,治疗组在此基础上加用MT,治疗8周。分别于治疗前后测定桡侧腕伸肌主动收缩时的最大肌电(EMG)值、手指(除拇指)共同屈伸总主动关节活动范围(TAM)、Fugl—Meyer评估表(FMA)的手功能评分(包括腕背伸、屈曲、环转运动、勾状抓握、侧屈等)。结果治疗前两组患者的各项评价指标评定差异无统计学意义(P〉0.05);治疗8周后,两组患者上述指标较治疗前均有所改善(P〈0.01),且治疗组的改善程度较对照组更明显(P〈0.05)。结论MT结合EMGBF有助于改善脑卒中偏瘫患者的手功能。Objective To investigate the effects of mirror therapy (MT) combined with electro- myographic biofeedback (EMGBF) on hand function in patients with stroke. Methods Sixty stroke pa- tients with hemiplegia were randomly divided into control group (n = 30) and treatment group (n = 30). All patients received routine comprehensive rehabilitation treatment and EMGBF for 8 weeks. The treat- ment group received MT in addition. The maximum EMG values of extensor contraction of wrist dorsi- flexion activity, total active movement(TAM) of finger (except the thumb) joint, Fugl--Meyer Assess- ment(FMA) used to assess hand function (include wrist dorsiflexion, flexion, rotation movement, hook grip, lateral bending, etc. ) were assessed before and 8 weeks after treatment. Results There was no significant difference on all scores between two groups before treatment. After treatment for 8 weeks, EMG, TAM and FMA scores were significantly increased in two groups as compared with pretreatment (P 〈 0.01). Moreover, the scores in the treatment group were increased more significantly (P 〈 0.05). Conclusions The treatment of MT combined with EMGBF contributes to the improvement of hand func- tion in hemiplegic patients.
