补肾健脾方联合常规降糖治疗男性脾肾两虚2型糖尿病不育多中心随机平行对照研究

摘　　要：[目的]观察补肾健脾方联合常规降糖治疗男性脾肾两虚2型糖尿病不育疗效。[方法]使用多中心随机平行对照方法,将73例门诊患者按就诊顺序编号抽签简单随机分为两组。对照组36例常规降糖治疗。治疗组37例补肾健脾方(淫羊藿12g,鹿角胶15g,黄精12g,沙苑子、制首乌各15g,黄芪、山药、葛根、丹参各30g、制大黄10g),1剂/d,水煎至200m L,温服,1次/d;常规降糖治疗同对照组。连续治疗90d为1疗程。观测临床症状、随机血糖、空腹血糖、糖耐量试验(OGTT)、糖化血红蛋白、前向运动精子百分率、正常精子形态率、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈18例,显效9例,有效4例,无效6例,总有效率83.8%;对照组痊愈6例,显效3例,有效1例,无效26例,总有效率28.0%;临床疗效治疗组优于对照组(P<0.01)。糖化血红蛋白两组均有改善(P<0.01),组间无明显差异(P>0.05);前向运动精子百分率两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01);正常精子形态率两组均有改善(P<0.01,P<0.05),治疗组改善优于对照组(P<0.05)。[结论]补肾健脾方联合常规降糖治疗男性脾肾两虚2型糖尿病不育,疗效满意,无严重不良反应,值得推广。[Objective]Abserve the effection on semen parameters of male infertility with type 2 diabetes by the decoction of nourishing kidney and tonifying spleen.[Methods]Randomly divide 73 Infertile male patients with type 2 diabetes mellitus into treatment group 37 patients and control group 36 patients with multi center,the control group is lowering the blood glucose by conventional therapy,the treatment group is lowering the blood glucose by conventional therapy and the decoction of nourishing kidney and tonifying spleen（Yinyanghuo12g,Lujiaojiao15 g,Huangjing12g,Shayuanzi、Zhishouwu15g,Huangqi、Shanyao、Gegen、Danshen30g、Zhidahuang10g）,once a day,till 200 ml water;routine hypoglycemic treatment with the control group.Continuous treatment of 90 d to 1 course of treatment.Observation of clinical symptoms,random blood glucose,fasting glucose,glucose tolerance test（OGTT）,glycosylated hemoglobin,forward movement sperm percentage,rate of normal sperm morphology and adverse reactions.As a course treatment,judge curative effect.[Results]The glycated hemoglobin is lowering significantly in both groups after 3 months treatment（P〈0.01）,and there is no difference between the treatment group and the control group（P〉0.05）;The percentage of sperm forward movement is improved significantly in both groups after 3 months treatment（P〈0.01）,and the treatment group is better than control group（P〉0.01）,The percentage of normal sperm morphology is improved significantly in both groups after 3 months treatment（P〈0.01,P〈0.05）,and the treatment group is better than control group（P〈0.05）.[Conclusion]The decoction of nourishing kidney and tonifying spleen can improve the effection on semen parameters of male infertility with type 2 diabetes significantly without serious adverse reactions.The curative effect is satisfied,and worth to be extended.
