培土滋阴稳眩汤联合银杏达莫治疗椎-基底动脉供血不足性眩晕随机平行对照研究

摘　　要：[目的]观察培土滋阴稳眩汤联合银杏达莫治疗椎-基底动脉供血不足性眩晕疗效。[方法]使用随机平行对照方法,将64例住院/门诊患者按病志号抽签/就诊顺序号方法简单随机分两组。对照组32例银杏达莫20m L+0.9%氯化钠注射液250m L,1次/d,静滴。治疗组32例培土滋阴稳眩汤(党参、茯苓、白术、何首乌制各15g,山药30g,丹参、山茱萸、半夏、陈皮、僵蚕、葛根、甘草各10g;呕吐加竹茹10g,旋覆花5g;失眠加夜交藤、合欢皮各15g,龙骨20g),1剂/d,水煎400m L,早晚口服;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、椎基底动脉血流、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈10例,显效12例,有效7例,无效3例,总有效率90.63%。对照组痊愈7例,显效8例,有效7例,无效10例,总有效率68.75%。治疗组疗效优于对照组(P<0.05)。椎-基底动脉血流治疗组改善优于对照组(P<0.01)。[结论]培土滋阴稳眩汤联合银杏达莫治疗椎-基底动脉供血不足性眩晕,疗效满意,无严重不良反应,值得推广。[Objective]To observe the effect of Peitu Ziyin Wenxuan decoction in the treatment of vertebralbasilar artery insufficiency vertigo.[Methods]64 patients were randomly divided into treatment group and control group,each of 32 cases.Control group was given Ginkgo Leaf Extract and Dipyridamole injection 20 ml into 0.9% sodium chloride injection 250 ml intravenous infusion once a day.On the basis of the treatment in the control group,treatment group is given extra Peitu Ziyin Wenxuan decoction（Codonopsis pilosula/ poria cocos/ atractylodes macrocephala/ Polygonum multiflorum/ Chinese yam/ salviae miltiorrhizae/ cornus officinalis/ pinellia ternate/Bombyx Batryticatus/ Pueraria/ radix glycyrrhizae）.1 agent/day,taken night and morning.Two groups both have a treatment for 14 days to evaluate the effect.Observe the clinical vertigo symptoms and the result of Transcranial Doppler（TCD） before and after treatment for the two groups.[Result]Results of the treatment group：10 cases got clinical cure,12 cases were significantly effective,7 cases were effective and 3 cases were noneffective.And the results of the control group ： 7 cases got clinical cure, 8 cases were significantly effective,7 cases were effective and 10 cases got failure.The total effective rate of the treatment group was 90.63% which is higher than that of the control group 68.75%（P〈0.05）.The treatment group improved more than control group on vertebral basilar artery blood flow（P〈0.01）.[Conclusion]Peitu Ziyin Wenxuan decoction has significant effect in the treatment of vertebral-basilar artery insufficiency vertigo.
