眼底激光联合曲安奈德治疗糖尿病性视网膜病变的临床研究

摘　　要：目的探讨眼底激光联合曲安奈德治疗糖尿病性视网膜病变（diabetic retinopathy,DR）的临床效果。方法单眼DR患者102例,分为两组。观察组,选取2012年10月至2013年10月单眼DR患者53例,采用眼底激光手术联合玻璃体腔注射曲安奈德治疗,激光手术治疗前1周给予曲安奈德注射;对照组,选取2011年9月至2012年9月单眼DR患者49例,单纯应用眼底激光治疗,光斑直径200~300μm,曝光时间0.1~0.2 s,初始治疗功率为100 m W。术后1周,1、3和6个月进行随访,观察两组患者治疗后视力水平、眼底改善及并发症等情况。结果观察组53例,视力提高率为67.9%,视力下降率为13.2%,疗效好于对照组,两组比较差异具有显著意义（P〈0.05）。治疗后观察组患者的眼底出血、渗出、视网膜水肿与黄斑区视网膜厚度均明显优于对照组,具有统计学意义（P〈0.05）。结论眼底激光联合曲安奈德联合治疗DR,可明显改善患者的视网膜相关症状,对其视力恢复具有良好作用。Objective To analyze the clinical effects of joint application of laser and triamcinolone acetonide on diabetic retinopathy.Methods Altogether 102 DR patients were divided into two groups： the observation group with 53 DR patients given combined treatment of laser and triamcinolone acetonide in our hospital from Oct. 2012 to Oct. 2013,and the control group with 49 DR patients treated solely with laser in our hospital from Sep. 2011 to Sep. 2012. While the patients of the control group being treated with fundus laser,those of the observation group were given intravitreal injection of triamcinolone acetonide before laser irradiation. The visual acuity,fundus improvement and complications after treatment of the two groups were observed and compared.Results As to the observation group,the improvement rate of visual acuity was 67. 9% and the decrease rate 13. 2%. The observationgroup boasted better vision conditions than the control group,indicating differences that were statistically significant（ P 0. 05）. Moreover,the observation group also performed better in hemorrhage,exudate,edema and macular thickness than the control group,and the differences were significant（ P 0. 05）.Conclusions Combined treatment of laser and triamcinolone acetonide on diabetic retinopathy may improve symptoms of retinal remarkably and promote visual recovery.
