红外偏振光联合He-Ne激光治疗头面部带状疱疹后遗神经痛的疗效观察

摘　　要：目的探讨红外偏振光星状神经节照射联合He-Ne激光治疗头面部带状疱疹后遗神经痛（postherpetic neuralgia PHN）的疗效。方法头面部带状疱疹后遗神经痛患者28例,采用红外偏振光星状神经节照射,光斑7 mm,照射强度为最大功率80%~90%,照射时间2 s,间歇时间3 s,每次共照射10 min,每日1次,20次为1个疗程,治疗2个疗程,每个疗程间歇10 d。He-Ne激光痛区照射,治疗功率30 m W,光斑大小2~10 cm,照射时间15 min,每日1次,20次为1个疗程,治疗2个疗程,每个疗程间歇10 d。治疗前后应用视觉模拟评分（visual analogue scale VAS）评价疼痛、并进行世界卫生组织生存质量测定简表（quality of life-brief,WHOQOL-BREF）评分,比较治疗前后评分变化。治疗后3和6个月随访。结果 28例患者治疗1个疗程后,VAS评分低于治疗前（P〈0.01）,WHOQOL-BREF评分高于治疗前（P〈0.01）,治疗前后比较差异有非常显著意义。治疗2个疗程后VAS评分更低、WHOQOL-BREF评分更高,与治疗1个疗程相比,差异有非常显著意义（P〈0.01）。结论采用红外偏振光星状神经节照射联合He-Ne激光痛区照射治疗可减轻头面部带状疱疹后遗神经痛,改善患者生活质量。Objective To evaluate the effect of the combined application of infrared polarized irradiation on stellate ganglion and helium-neon laser for the treatment of craniofacial postherpetic neuralgia patients. Methods Twenty-eight patients with craniofacial postherpetic neuralgia, infrared polarized irradiation on stellate ganglion, 7 mm spot,illuminate intensity is 80% - 90% of maximum output power, and irradiationt 2 s, intermittent time 3 s each once, irradiation 10 min a day, 20 times for a period of treatment, two courses of treatment, the treatment course intermittent 10 d. He-No laser irradiation pain area, illuminate power 30 mW, pot size 2 to 10 cm, irradiation 15 min a day, 20 times for a period of treatment, two courses of treat- ment, the treatment course intermittent 10 d. Before and after treatment using visual analogue scale （visual analogue scale VAS） evaluation of pain, and the world health organization quality of life measurement profile （quality of life-brief, WHOQOL BREF） score, compare with the score changes before and after the treatment. After treatment 3 and 6 months follow-up the patients. Results After 1 and 2 courses of treatment, the VAS scores dropped （ P 〈 0. 01 ）, but the WHOQOL-BREF scores rose （P 〈 0. 01 ）. Compared with those after 1 course of treatment, the VAS scores after 2 courses of treatment was lower and the WHOQOL-BREF scores higher, and the difference was statistically siguifioant （P 〈0. 01 ）. Conclusions The combination of infrared polarized radiation on stellate ganglion and He-Ne laser irradiation on pain areas of craniofa- cial postherpetic neuralgia may reduce pain and improve quality of life.
