光动力疗法在门诊眼科的临床应用和护理配合

摘　　要：目的探讨光动力疗法(Photodynamic therapy,PDT)在门诊眼科的临床应用及其疗效,评价护理配合及健康指导避光措施等在治疗过程中的作用。方法回顾性分析,2014年3月至2015年3月脉络膜新生血管疾病患者30例30只眼的临床资料,给予PDT治疗,对患者实施PDT治疗前后相应的分层护理措施,观察患者的临床疗效和并发症发生率。结果 30例患者按照标准规程完成激光治疗,无静脉渗漏及药物剂量偏差,均行视力、眼压等专科检查确诊,无并发症发生。1个月后门诊复查,29例29只眼视力提高2行以上,1例1只眼视力无明显改善。3个月后视力保持稳定,无明显回退。结论护理工作的操作规范性、切实有效的护理措施和避光指导是确保PDT治疗成功、降低不良反应的重要部分。Objective To investigate the curative effect of photodynamic therapy on outpatient eyes,and assess the role of nursing cooperation and light protection measures.Methods Totally 30 outpatients（ 5 females and 15 males） given photodynamic therapy were retrospectively reviewed for nursing experience. Their ages ranged from 27 to 73 years old and averaged（ 49. 5 ± 12. 9） years. For definitive diagnosis,the vision,intraocular pressure,fundus fluorescein angiography（ FFA） and optical coherence tomography（ OCT） were obtained for all these patients before the treatment. Photodynamic therapy was conducted to treat polypoidal choroidal vasculopathy（ PCV,one patient）,central serous chorioretinopathy（ CSC,8patients）,pathologic myopia choroidal neovascularization（ PM CNV,2 patients） and wet aging macular degeneration（ WAMD,19 patients） respectively. Stratified nursing measures were carried out before and after the treatment.Results Altogether 30 patients（ 30 eyes） received laser treatment in accordance with standard procedures. Venous leakage,doses deviation,and complication did not occur to any patient. Except of a patient who had no evident improvement,visual acuity increased by more than 2 rows in 29 patients a month after the outpatient care. The vision remained stable and no obvious relapse occurred 3 months later.Conclusions The keys to ensure the success of photodynamic therapy include performance of normative nursing work,effective nursing measures and light protection guidance.
