摘  要:目的探讨HCV感染与血小板抗原基因之间的关系。方法提取114例抗-HCV ELISA双试剂检测均有反应性的献血者血清标本的RNA,做HCV RNA荧光定量检测,随访后将其分为HCV持续感染组(39例)和HCV自限清除组(75例);提取人基因组,用PCR-SSP方法对人血小板抗原(HPA)基因进行分型;比较HCV感染献血者与本地合格献血者和其他地区合格献血者HPA基因分型情况,分析HCV感染与HPA分型之间的关联性。结果与合格献血者相比,HCV感染献血者的HPA分型有不同表现(P>0.05);HPA基因多态性在HCV感染不同转归献血者中明显不同(P<0.05)。HCV持续感染组和对照组相比,HPA等位基因多态性有明显差异(P>0.05);HCV自限清除组和对照组的HPA-3a和HPA-3b基因频率分别为:40.70%vs 61.00%和59.30%vs 39.00%(P<0.01)。结论HPA-3的基因多态性与HCV易感性和自限清除是否具有一定的关联性,值得进一步研究。Objectives To determine the allele frequency of the HPA-1 to 18 in blood donors infected with HCV and to investigate the relationship between HCV infection and the polymorphism of HPAs. Methods Blood samples were collected from 141 blood donors who were screened anti-HCV positive by two ELISA agents. HCV RNA was extracted and qualified by fluorogenic quantitative PCR when donors were followed up. According to the results,two groups were divided into persistent infection and spontaneous clearance. Human DNA was isolated and amplified for genotype of HPA-1- 18 using PCR Sequence Specific Primers( PCR-SSP) kits. HPAs polymorphism was compared between blood donors infected with HCV and eligible blood donors( local and other regions.). The association between HCV infection and HPAs polymorphism was analyzed. Results The allelic and genotypic frequencies of HPAs between HCV infection and eligible blood donors were not significantly different( P〉 0. 05). There was no significant difference between HCV persistent infection and eligible groups.The frequencies of HPA-3a and HPA-3b in spontaneous clearance groups and eligible groups were 40. 7% vs 61. 00% and59. 30% vs 39. 00%,respectively,showing significantly difference( P〈 0. 01). Conclusion HPA-3 allelic frequency in HCV infection may indicate a possible association between HCV infection and clearance and HPAs,but further research is needed.
