带旋髂深血管蒂髂骨块移植治疗胫骨大块骨缺损的疗效观察

摘　　要：目的 研究带旋髂深血管蒂髂骨块移植治疗胫骨大块骨缺损的疗效.方法 28例胫骨大块骨缺损患者, 取带旋髂深血管蒂髂骨块移植到胫骨骨缺损处, 游离血管与临近下肢血管吻合提供血供.观察治疗效果.结果 对所有患者随访12~24个月, 其中显效11例, 占39.29%, 有效16例, 占57.14%, 无效1例, 占3.57%, 总有效率为96.43%.结论 带旋髂深血管蒂髂骨块移植治疗胫骨大块骨缺损, 具有改善骨折处血液循环、加速骨愈合等优点, 值得临床推广.Objective To research curative effect by transplantation of iliac flap with deep circumflex artery pedicle in the treatment of massive bone defect in tibia.Methods A total of 28 patients with massive bone defect in tibia received transplantation of iliac flap with deep circumflex artery pedicle to defect site, with blood supply by anastomosis of free blood vessel and close lower limb blood vessel. Their curative effects were observed. Results Follow-up lasting 12~24 months for all patients showed 11 excellent cases, accounting for 39.29%, 16 effective cases, accounting for 57.14%, and 1 ineffective case, accounting for 3.57%. The total effective rate was 96.43%.Conclusion Implement of transplantation of iliac flap with deep circumflex artery pedicle in the treatment of massive bone defect in tibia contains advantages of improving blood circulation and accelerating bone healing. This method is worthy of clinical promotion.
