合并鼻中隔偏曲的难治性鼻出血21例治疗体会

摘　　要：目的探讨合并鼻中隔偏曲的偏曲侧难治性鼻出血的临床治疗方法。方法回顾性分析21例合并鼻中隔偏曲的偏曲侧难治性鼻出血患者的临床资料,对所有患者在全身麻醉下行鼻中隔偏曲矫正术后鼻内镜下止血,观察临床疗效。结果 21例患者中15例鼻内镜检查明确出血部位低温射频止血成功,6例鼻内镜检查未能明确出血部位,行碘仿纱条前鼻孔填塞后未再出血。结论鼻中隔偏曲合并偏曲侧难治性鼻出血应尽早行全身麻醉下鼻中隔偏曲矫正术后鼻内镜下止血,可减轻患者痛苦,止血效果好,值得临床推广应用。Objective To investigate clinical treatment method for intractable epistaxis complicated with deviation of nasal septum.Methods Clinical data of 21 intractable epistaxis patients with complicated deviation of nasal septum were retrospectively analyzed. All patients received nasal endoscopic hemostasis after diorthosis for deviation of nasal septum under general anesthesia, and their clinical effects were observed.Results Among 21 patients, 15 cases received successful low temperature radiofrequency hemostasis in diagnosed hemorrhage position under nasal endoscope, and the other 6 cases received hemostasis by tela iodoformum blocking for unclear hemorrhage position.Conclusion It is necessary for early implement of nasal endoscopic hemostasis after diorthosis for deviation of nasal septum under general anesthesia for intractable epistaxis patients with complicated deviation of nasal septum. This method can relieve pain with good hemostasis effect, and it is worthy of clinical promotion and application.
