间歇式充气加压预防直肠癌术后下肢深静脉血栓的效果研究

摘　　要：目的探讨间歇式充气加压预防直肠癌术后下肢深静脉血栓形成的价值。方法 2013年1月至2014年12月纳入直肠癌284例,随机分为观察组和对照组各142例。观察组术后使用空气波压力治疗仪给予双下肢间歇式充气加压护理,每次30min,2次/d,连用7d;对照组术后给予医用加压弹力袜和下肢常规护理干预。观察彩色超声多普勒检测术前、术后1、3、7d两组股静脉平均血流速度和血流量,以及下肢深静脉血栓形成和肺栓塞发生情况。结果术后3、7d,观察组股静脉平均血流速度(3d:25.9±4.0vs 24.0±4.0cm/s;7d:25.6±4.2vs 24.4±3.8cm/s)和血流量(3d:14.8±3.2vs 12.6±2.8mL/s;7d:14.6±3.0vs 12.8±2.6mL/s)显著高于对照组(P<0.05)。观察组术后30d下肢深静脉血栓形成发生率(2.1%vs7.0%,P<0.05)显著低于对照组。两组均无肺栓塞发生。结论间歇式充气加压有助于改善下肢深静脉血流速度和血流量,是预防直肠癌术后下肢深静脉血栓形成安全可靠的护理方法,效果优于医用加压弹力袜。Objective To evaluate the prevention of intermittent pneumatic compression on deep vein thrombosis after operation in patients with rectal cancer. Methods During the period from January 2013 to December 2014,284 cases of rectal cancer patients were prospectively enrolled and randomly divided into experimental group and control group （142 cases in each group）. The experimental group were given inter- mittent pneumatic compressionfor double lower limbs with air wave pressure treatment instrument （30rain each time,2 times/d,during 7 days}. The control group were given medical pressure cycling socks for lower limbs and routine nursing after operation. The femoral vein average blood flow velocity and blood flow were measured by Color Doppler ultrasound on preoperative and postoperative 1,3, and 7 day. The lower limb deep venous thrombosis incidence were also detected by Color Doppler ultrasound. Results The post- operative 3 and 7d,the mean velocity of blood flow of the femoral vein （3d：25.9±4 vs 24±4.0cm/s; 7d： 25.6±4.2 vs 24.4±3.8 cm/s） and blood flow （3d：14.8 3.2 vs 12.6±2.8 ml/s; 7d：14.6±3 vs 12.8±2.6 ml/s） in the experimental group was significantly higher than that in control group （ P 〈0.05）. The lower extremity deep venous thrombosis incidence [2.1% vs 7.0%, P 〈0. 05） in the experimental group was significantly lower than the control group. Conclusion The intermittent pneumatic compression helps to improve lower extremity deep venous blood flow velocity reliable method for preventting the lower extremity deep and be better than medical pressure cycling socks. and blood flow,Suggesting that it is a safe and venous thrombosis after rectal cancer operation
