三桑肾气汤联合西药治疗肾气虚型慢性阻塞性肺疾病随机平行对照研究

摘　　要：[目的]观察三桑肾气汤联合西药治疗肾气虚型慢性阻塞性肺疾病疗效。[方法]使用随机平行对照方法,将140例住院及门诊患者按抛硬币法简单随机分为两组。对照组70例Ⅰ级:沙丁胺醇,咳嗽、咳痰或呼吸困难不能自抑时喷1次,仍不缓解加喷1次,<8次/d;II级、III级、IV级:沙丁胺醇,咳嗽、咳痰或呼吸困难不能自抑时喷1次,仍不缓解加喷1次,<8次/d;沙美特罗替卡松,1喷/次,2次/d。氧疗,2L/min,16h/d。治疗组70例三桑肾气汤(桑椹^(川、晒干)、桑寄生^(粤、晒干)、桑白皮^(蜜炙)、补骨脂^(豫、盐炙)、杜仲^(川、盐炙)各9g,女贞子^(川、酒炙)、黄精^(冀、酒炙)各12g;肺气虚加黄芪^(内蒙古、蜜炙)12g,白术~浙、防风^(冀、晒)干各9g;脾气虚加党参^(川、晒干)、茯苓~云各12g,甘草^(内蒙古、蜜炙)6g,陈皮^(粤、晒干)、半夏^(川、姜炙)、白术^(麸炒)各9g;肝气郁滞加柴胡^(冀、醋炙)、前胡^(浙、蜜炙)、赤芍^(川、晒干)、白芍^(浙、酒炒)各9g,青皮^(粤、醋炙)、陈皮^(粤、晒干)各6g),1剂/d,水煎300m L,早晚口服,150m L/次;西药治疗同对照组。连续治疗90d为1疗程。观测临床症状、6分钟步行距离、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效26例,有效33例,无效11例,总有效率84.29%。对照组显效8例,有效40例,无效22例,总有效率68.57%。治疗组疗效优于对照组(P<0.05)。治疗组6分钟步行距离有改善(P<0.05),对照组无改善(P>0.05),治疗组优于对照组(P<0.05)。[结论]三桑肾气汤联合西药治疗肾气虚型慢性阻塞性肺疾病疗效满意,无严重不良反应,值得推广。[Objective]To observe the three joint Sang Shenqitang western medicine kidney deficiency type of chronic obstructive pulmonary disease treatment.[Methods]Randomized parallel controlled method,140 inpatients and outpatients by disease Zhi No.lottery method is simple and randomly divided into two groups.The control group 70 cases of gradeⅠ：salbutamol,cough,sputum 1 spray or breathing difficulties can not self-suppression,is still not alleviate plus 1 spray,8 times/d;II level,III level,IV grade： salbutamol,cough,expectoration,or dyspnea can not spray a second time since the suppression has failed to ease plus 1 spray,8 times/d;salmeterol and fluticasone propionate,1 spray/times,2 times/d.Oxygen therapy,2L/min,16h/d.The treatment group 70 cases three Sang Shen Qi Tang （Sangshen^（chuan,Shaigan）Sangjisheng^（Yue,Shaigan）,Sangbaipi^（Mizhi）,Buguzhi^（Yu,yanjiu）,Duzhong^（Chuan,Yanzhieach） 9g,Nvzhenzi^（Chuan,jiuhzi）,Huangjing^（Ji,jiuzhi）each 12g;lung qi Huangqi^（Neimenggu,Mizhi）12g,Baizhu^（Zhe）,Fangfeng^（Ji,Shaigan） each 9g;spleen qi deficiency plus Dangshen^（Chuan,Shaigan）,Fuling^（yun）each 12g,Gancao^（Neimenggu,mihzi） 6g,Chenpi^（Yue,shaigan）,Banxia^（Chuan,jiangzhi）,Baizhu^（Fuchao） 9g;qi stagnation plus Chaihu^（Ji,cuzhi）,Qianhu^（zhe,mizhi）,Chishao^（Chuan,Shaigan）,Baishao^（Zhe,Jiuchao）each 9g,Qingpi^（yue,cuzhi）,Chenpi^（Yue,Shaigan）ea ch 6g）,1 ji/d,decoction 300 m L,sooner or later,oral,150 m L/times.Western medicine with the control group.90 d for a course of continuous treatment.Observation of clinical symptoms,6 minutes walking distance,adverse reactions.One course of treatment,to determine efficacy.[Results]The treatment group 26 cases,effective 33 cases,ineffective in 11 cases,the total efficiency of 84.29%.A control group that were 8 cases,effective 40 cases,22 cases ineffective,the total efficiency of 68.57%.Treatment group than the control group（P0.05）.6-minute walk from the treatment group improved（P0.05）,no impro
