摘  要:目的探讨三维超声容积对比成像C平面(VCI-C)技术在胎儿颅后窝病变鉴别诊断中的应用价值。方法应用VCI-C平面技术定量分析二维超声诊断胎儿颅后窝病变18例(典型Dandy-Walker畸形3例,Dandy-Walker变异型7例,Blake囊肿2例,颅后窝蛛网膜囊肿2例,颅后窝池增大4例),测量胎儿小脑蚓部面积、脑干-小脑蚓部夹角(BV)、脑干-小脑幕夹角(BT),与二维超声诊断结果相对照。结果 (1)3例典型Dandy-Walker畸形小脑蚓部缺失或极小,BV、BT角明显增大,小脑幕上抬。(2)7例Dandy-Walker变异型中:2例是小脑蚓部发育不良,1例是小脑蚓部发育不全,均BV、BT角增大;4例小脑蚓部发育正常,BV、BT角正常或轻度增大。(3)2例Blake囊肿、2例颅后窝蛛网膜囊肿、4例颅后窝池增宽均小脑蚓部发育正常,BV、BT角不同程度变化。结论 VCI-C平面技术有助于胎儿颅后窝病变的准确诊断,应作为产前诊断与鉴别诊断胎儿颅后窝病变的常规检查方法。Objective: To investigate the value of 3D ultrasound volume contrast imaging-C (VCI-C) in fetal posterior fossa lesions in the differential diagnosis. Methods: Quantitative analysis by VCI-C technology the 2D ultrasound diagnosis of fetal posterior fossa lesions 18 cases (3 cases Dandy-Walker malformation: 7 cases Dandy-Walker variant; 2 cases Blake's cyst; 2 cases posterior fossa arachnoid cyst; 4 cases posterior fossa pool increase) .Measured the area of cerebellar vermis, the BV angle and BT angle. Results: ( 1 ) 3 cases of Dandy-Walker malformation deformity of cerebellar vermis absent or very small, BV, BT angle increases obviously, lift the tentorium cerebelli. (2) In 7 cases Dandy-Walker variant: 2 cases is vermian hypoplasia, 1 case is vermian agenesis, all BV, BT Angle increases. (3) 2 cases Blake's cyst, 2 cases posterior fossa arachnoid cyst, 4 cases posterior fossa pool increase are all normal cerebellar vermis development, BV, BT Angle changes in different level. Conclusion: VCI-C technology help the accurate diagnosis of fetal posterior fossa lesions, VCI-C technology can be routinely used for prenatal diagnosis and differential diagnosis of fetuses with posterior cranial fossa lesions.
