城区老年人躯体疾病、生活事件与抑郁症相关性调查研究

摘　　要：目的探讨城区老年人躯体疾病、生活事件对抑郁症的影响。方法采用方便抽样的方法,抽取十堰市500名城区老年人,选用老年精神状况量表及与之相配套的计算机诊断系统调查并诊断抑郁症,选用老年健康状况MDS和相关因素调查表调查躯体疾病和生活事件,并用Logistic回归分析、因子分析等方法进行统计学分析。结果老年抑郁症患者心绞痛、中风或身体部分偏瘫、偏头痛、视力问题、甲状腺功能亢进或甲状腺功能减退的患病率及躯体疾病数目都显著高于非抑郁症患者。中风或身体部分瘫痪OR(95%CI)=6.389(1.684,24.237)、情感受创事件OR(95%CI)=1.438(1.069,1.934)、健康经济不利事件OR(95%CI)=1.652(1.099,2.483))是老年人抑郁症的危险因素。结论一些躯体疾病和生活事件可能影响了城区老年人抑郁症的发病过程,需要采取多方面的预防措施降低躯体疾病、生活事件对抑郁症的风险。Objective To explore the influence of somatic diseases and life events on depression of the elderly urban people.Methods 500 elderly urban people in Shiyan City were investigated by Convenience Sampling method, using the Geriatric Mental state Schedule with the support of Automated Geriatric Examination for Computer Assisted Taxonomy for the investigation and diagnosis of depression and Geriatric Health Condition Minimum Data Set and Related Factors Schedule for the investigation of somatic diseases and life events among elderly people. Data were analyzed via logistic regression analysis, factor analysis and other statistical methods. Results The elderly with depression had a significant higher prevalence rate of angina, stroke or hemiplegia part,migraine, vision problems and hyperthyroidism or hypothyroidism than those without depression as well as a significant larger number of somatic diseases. The risk factors of elderly depression were stroke or hemiplegia OR（95%CI）=6.389（1.684, 24.237）,emotional trauma event OR（95%CI）=1.438（1.069, 1.934） and adverse health or economic events OR（95%CI）=1.652（1.099, 2.483）.Conclusion Some somatic diseases, life events may influence the developing process of depression in the elderly, and all-round preventative measures should be adopted to reduce risks of somatic diseases and life events on depression.
